丹参注射液联合阿法骨化醇对糖尿病肾病患者血清HbA1c、TGF-β、α1-MG水平变化及血液流变学的影响

摘　　要：目的:观察丹参注射液联合阿法骨化醇对2型糖尿病肾病(T2DN)患者血清转化生长因子-β(TGF-β)、α1-微球蛋白(α1-MG)、糖化血红蛋白(HbA1c)水平变化及血液流变学的影响。方法:84例T2DN患者,随机数字表法分为对照组与观察组,常规干预基础上对照组口服阿法骨化醇治疗,观察组在对照组基础上加用丹参注射液治疗,两组均持续治疗1个月;疗程结束后统计两组临床疗效。比较治疗前后血清TGF-β、α1-MG、HbA1c水平及血液流变学指标(红细胞压积、纤维蛋白原、血浆黏度)变化,统计不良反应发生率。结果:观察组总有效率(92. 9%)较对照组(71. 4%)高,差异有统计学意义(P<0. 05);治疗后两组血清TGF-β、α1-MG、HbA1c水平较治疗前降低,且观察组TGF-β、α1-MG、HbA1c水平较对照组低,差异有统计学意义(P <0. 05);治疗后两组红细胞压积、纤维蛋白原、血浆黏度水平较治疗前降低,且观察组较对照组明显,差异有统计学意义(P <0. 05);观察组不良反应发生率(14. 3%)与对照组(9. 5%)比较,差异无统计学意义(P> 0. 05)。结论:联合应用阿法骨化醇及丹参注射液治疗2型糖尿病肾病效果显著,可有效降低血清TGF-β、α1-MG、HbA1c水平,改善血液流变学。Objective:To investigate the effects of Danshen Injection combined with Calcitriol on serum lev- els of TGF-β,αl-MG,HbA1c and hemorheology in patients with type 2diabetic nephropathy.Methods:84 patients with T2DN were randomly divided into.control group and observation group ,On the basis of routine in- tervention,the control group was treated with Calcitriol,and the observation group was treated with Danshen In- jection combined with Calcitriol.The two groups were treated continuously for 3 months.After the treatment, making statistical comparison of the clinical efficacy of the two groups,serum TGF β,αl-MG ,HbA1c levels and blood rheology index (hematocrit,fibrinogen,plasma viscosity)were determined before and after treatment, and the incidence of adverse reactions was recorded.Results:The total effective rate of the observation group (92.9%)was higher than that of the control group (71.4%),and the difference was statistically significant (P <0.05).After treatment,the serum TGF-β,αl-MG and HbAlc levels were decreased in the two groups , the levels in the observation group after treatment were lower than those of control group (P <0.05);the hema- tocrit,fibfinogen and plasma viscosity of the two groups were improved significantly after treatment,those of the observation group were lower than those of the control group,and the difference were statistically significant (P<0.05).The incidence of adverse reactions in the observation group(14.3%)was not statistically significant compared with the control group (9.5%)(P >0.05).Conclusion:Danshen Injection combined with Caleitriol can effectively treat DN,reduce serum TGF-β,αl-MG,HbAle levels and improve blood rheology.
