摘 要:目的探讨3.0TMR扩散张量成像(DTI)对膝部神经三维重建的应用价值。方法对22例志愿者左膝部行DTI序列扫描,扩散敏感梯度数(MPGs)为25(MPGs=25);选垂直方向原始图行最大密度投影(MIP)、容积再现(VR)和扩散张量纤维束成像(DTT)重建。由2名医师行可视化评价。对于同一条神经的评分,MIP、VR和DTT之间采用Friedman检验,利用组内相关系数(ICC)分析比较3种方法的相关性。2位观察者之间评分一致性采用Kappa统计方法分析。结果对同一条神经,VR、MIP与DTT三者成像质量评分无明显统计学差异(P>O.05)。ICC值为O.718~0.856,Ⅳ值为0.668~O.705。MIP、VR重建较DTT重建时间短、操作简易。结论应用VR、MIP、DTT3种方法对DTI原图重建,均能得到立体、清晰的膝部胫、腓总神经。VR、MIP重建较DTT重建简易高效、客观性强,可以作为繁琐、主观性强的DTT重建的有效前期预览手段。Objective To explore the application value of 3.0T MR diffusion tensor imaging (DTI)in 3D reconstruction of the knee peripheral nErve.Methods DTI with 25motion probing gradients (MPGs)was performed on the left knee in 22healthy young volunteel:s;By choosing the original images scaned in the vertical direction,three post-processing methods including maximum intensity projection (M J P),volume rerkiering (VR)and diffusion tensor tractography (DTT)were used.Two radiologists evaluated these images visually.For the same nerve,MIP,VR and DTT were compared using Friedman test.Intraclass correlation coefficient (ICC)analysis was used to compare the correlation among the three imaging methods.Score consistency between two observers was calculated by using the Kappa statistic.Results For the same nerve,there was no significant statistical difference for the MIP,VR and DTT imaging quality scores(P>0.05).The ICC value and the k value were 0.718-0.856,0.668-0.705,respectively.Conclusion Stereoscopic and clear displaying the tibial nerve/the common peroneal nerve in the knee can be achieved by using MIP ,VR and DTT post-processing methods based on DTI original images.The reconstruction of MIP,VR is simple and efficient,in which the objectivity is better than DTT.The reconstruction of MIP,VR can be used as effective preview methods before the DTT reconstruction with complexity and strong subjective dependence.
