阻塞性睡眠呼吸暂停低通气综合征患者尿微量白蛋白检测的临床分析

摘　　要：目的测定阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者尿微量白蛋白(UMA)水平,探讨其在早期肾损伤评估中的价值。方法收集54例OSAHS患者及46例单纯鼾症患者的资料,收集并比较两组患者的临床指标。结果两组患者的年龄、性别、收缩压(SBP)、舒张压(DBP)无统计学差异(P>0.05)。OASHS组的呼吸暂停低通气指数(AHI)、 UMA值均显著高于单纯鼾症组,而夜间平均脉氧饱和度(MSaO_2)、夜间最低脉氧饱和度(LSaO_2)值均显著低于单纯鼾症组(P <0.05)。相关性分析显示OSAHS患者的UMA水平与AHI呈正相关,与MSaO_2呈负相关(P <0.05)。结论 OSAHS患者的UMA水平增高,提示存在早期肾脏功能损伤。Objective To detect the level of urine microalbumin (UMA)in patients with obstructive sleep apnea hypopnea syndrome (OSAHS),and to explore its value in early renal injury assessment.Methods 54cases of patients with OSAHS and 46cases of patients with simple snoring were enrolled in this study.The clinical data were collected and compared between the two groups.Results No statistical difference was found in the age,gender and blood pressure (SBP and DBP)between the two groups (P>0.05).The apnea hypopnea index (AHI)and LIMA level of the OSAHS group were significantly higher than those of the simple snoring group (P <0.05),while the mean oxygen saturation (MSaO2)and low oxygen saturation (LSaO2)level were significantly lower than those of the simple snoring group (P<0.05). Correlation analysis indicated that in OSAHS group,the UMA level was positively correlated with AHI and negatively correlated with MSaO2(P<0.05).Conclusions UMA level increases in patients with OSAHS,which suggests the early stage renal impairment.
