摘　　要：目的观察三氯异氰尿酸对多药耐药菌(MDRO)的杀灭效果,为临床合理选用消毒剂提供参考。方法观察三氯异氰尿酸对泛耐药鲍氏不动杆菌(XDRAB)、耐甲氧西林金黄色葡萄球菌(MRSA)、耐药铜绿假单胞菌(MDR-PA)、耐碳青霉烯类肺炎克雷伯菌(CRKP)及耐碳青霉烯类大肠埃希氏菌(CRECO)五种临床分离常见耐药菌的抗性实验,并对三氯异氰尿酸消毒效果进行临床评价。结果悬液定量杀菌试验结果显示浓度≥250mg/L的三氯异氰尿酸消毒液作用时间≥5min,可对XDRAB、MRSA、MDRPA、CRKP、CRECO均达到良好的杀灭效果;重症监护病房(ICU)临床评价结果表明,浓度≥500mg/L的三氯异氰尿酸消毒液可对操作按钮及病床达到合格的消毒水平。结论推荐临床使用浓度为500mg/L的三氯异氰尿酸消毒液进行医院环境高度风险区域物体表面擦拭消毒。OBJECTIVE To observe the killing effect of trichloroisocyanuric acid disinfectants on multidrug-resistant organisms(MDRO),so as to provide reference for clinical rational use of disinfectant.METHODS The killing effect of trichloroisocyanuric acid disinfectants on five common drug-resistant organisms in clinical isolates including extensively drug-resistant Acinetobacter baumannii(XDRAB),methicillin-resistant Staphylococcus aureus(MRSA),multidrug-resistant Pseudomonas aeruginosa(MDRPA),carbapenem-resistant Klebsiella pneumonia(CRKP)and carbapenem-resistant Escherichia coli(CRECO).Clinical evaluation was conducted for the disinfection effect of trichloroisocyanuric acid.RESULTS The suspension quantitative sterilization test results showed that trichloroisocyanuric acid disinfectants can effectively kill XDRAB,MRSA,MDRPA,CRKP,CRECO when the treatment time was longer than 5 min and the concentration was higher than 250 mg/L.Clinical evaluation results in ICU showed that trichloroisocyanuric acid disinfectants can effectively disinfect the buttons and hospital beds when the concentration was higher than 500 mg/L.CONCLUSION It is recommended to use 500 mg/L of trichloroisocyanuric acid disinfectants to clean the high-risk areas of hospital environments.
