摘 要:目的探讨骨科创口感染患者病原菌分布情况和变化趋势,为骨科创口感染的临床预防和相关实际研究提供依据。方法选取医院2014年1月-2016年10月骨科收治骨折患者877例为研究对象,患者入院接受治疗后详细询问、检查、记录患者性别、年龄、体质量、基础疾病等基本资料,检测患者的致病菌情况,并对各年的致病菌的变化情况进行总结和分析。结果 877例骨科患者中有152例发生创口感染,感染率为17.33%;2014-2016年间感染率水平呈下降趋势;152例创口感染患者共检出病原菌155株,其中革兰阳性菌39株占25.16%,革兰阴性菌78株占50.32%,真菌38株占24.52%;革兰阳性菌由2014年的26.92%降到2016年的22.64%,革兰阴性菌由2014年的46.15%升到2016年的54.72%,真菌由2014年的26.92%,降到2016年的22.64%。结论骨科患者创口感染情况较为严重,导致患者感染的病原菌以革兰阴性杆菌为主,2014-2016年间病原菌的种类、构成有所不同。OBJECTIVE To investigate the distribution and trend of change of pathogens in patients with orthopedic wound infection,and to provide evidences for the clinical prevention and related practical research of orthopedic wound infection.METHODS 877 patients with fractures treated from Jan.2014 to Oct.2016 in our hospital were enrolled in this study.Detailed inquiry,examination and recording of basic information such as gender,age,body weight,and underlying diseases were conducted for the patients admitted to the hospital.Pathogens were detected in the patients,and the changes of pathogens in each year were summarized and analyzed.RESULTS 152 cases of wound infection occurred in the 877 orthopedic patients,and the infection rate was 17.33%.The infection rate showed a decreasing trend from 2014 to 2016.Among the 132 patients with wound infection,155 strains of pathogens were detected,of which 39 were gram-positive bacteria,accounting for 25.16%,78 strains were gram-negative bacteria,accounting for 50.32%,and 38 strains were fungi,accounting for 24.52%.The proportion of gram-positive bacteria dropped from 26.92%in 2014 to 22.64%in 2016,and that of gram-negative bacteria increased from 46.15%in 2014 to 54.72%in 2016.The proportion of fungi dropped from 26.92%in 2014 to 22.64%in2016.CONCLUSION The wound infection in orthopedic patients is quite serious,and the pathogens that cause infection in the patients are mainly gram-negative bacteria.The types and composition of pathogens varied from 2014 to 2016.
