基于TOPSIS的2017年云南省法定传染病漏报情况综合评价及影响因素分析

摘　　要：目的为了解云南省各级各类医疗机构法定传染病漏报情况,对漏报情况做综合评价,并发现云南省传染病漏报的影响因素,从而进一步提高传染病报告和管理工作质量。方法对云南省6个地市31家不同级别的医疗机构进行法定传染病漏报调查,对定性及定量调查结果运用TOPSIS法进行综合评价,并采用Logistic回归分析对漏报影响因素进行分析。结果共调查16种法定传染病855例,漏报率为11.08%,其中乙类传染病11种,漏报率6.57%;丙类传染病4种,漏报率4.51%,乙、丙类传染病之间漏报率有统计学意义(χ^2=7.278,P<0.01)。综合评价结果显示,中部地区漏报情况较周边地区严重,经济发达地区较经济落后地区漏报情况严重,且随着医疗单位级别的升高,漏报情况逐渐增加。Logistic回归分析显示,漏报情况和医疗机构自查后奖惩情况相关。结论云南省法定传染病仍存在漏报情况,应从不同层面加强传染病漏报质量控制,不断降低传染病漏报率。Objective To understand the situation of missed reports of notifiable infectious diseases in various kinds of medical institutions in Yunnan province. To comprehensively evaluate the situation and find out the influence factors. To further improve the quality of infectious diseases report and the management. Methods The missed reports of notifiable infectious diseases in 31 different medical institutions from six cities of Yunnan province was investigated. The results of the investigation were evaluated comprehensively with the method of TOPSIS,the impact factors were analyzed by the method of Logistic regression. Results A total of 855 cases that were 16 kinds of notifiable infectious diseases were investigated. The rate of missed report was 11. 08%. Among them,there were 11 kinds of infectious diseases of class B,the rate of missed reports was 6. 57%; and there were 4 kinds of infectious diseases class C,the rate of missed reports was 4. 51%. The rates of missed reports of class B and class C had statistical significance ( χ^2 = 7. 278,P<0. 01) . The results of comprehensive evaluation showed that the situation of missed reports in the central region was more serious than the surrounding areas. The situation in the developed area was more serious than that in the undeveloped area. As the level of medical institution rose,the situation of missed reports was more seriously. The analysis of Logistic regression showed that the situation of missed reports was related to the situation of rewards and punishments after the self - examination of medical institution. Conclusion The missed reports of notifiable infectious disease were existed in Yunnan province. The quality control on this situation should be strengthened from different levels and so that to reduce the rate of missed reports of notifiableinfectious disease constantly.
