摘 要:目的探究大叶性肺部感染和肺炎型肺癌患者在CT影像下的特征对比。方法选取2013年6月-2017年6月于医院接受治疗的大叶性肺部感染患者69例和肺炎型肺癌患者54例,将大叶性肺部感染患者作为试验组,肺炎型肺癌患者作为对照组。检测大叶性肺部感染患者病原菌种类及构成比,对两组患者均进行CT检查,比较两组患者病灶数量及单发病灶分布情况,检查两组患者影像学特征(支气管充血、小结节影、瘤体显示和磨玻璃影环绕等)并进行比较。结果 69例大叶性肺部感染患者中共检出病原菌75株,其中革兰阴性菌49株占65.3%、革兰阳性菌24株占32.0%、其他菌种2株占2.7%;试验组患者单发病灶59例占85.5%、多发病灶10例占14.5%,对照组患者单发病灶45例占83.3%、多发病灶9例占16.7%,两组患者病灶数量分布比较差异无统计学意义;两组患者CT影响特征中支气管充血、小结节影、瘤体显示、磨玻璃影环绕、囊状透亮影指标比较差异均有统计学意义(P<0.05)。结论大叶性肺部感染与肺炎型肺癌病灶数量及分布情况基本无差别,CT影像特征支气管充血、小结节影、瘤体显示、磨玻璃影环绕和囊状透亮影有差异,检查时需注意鉴别以上CT影像特征。OBJECTIVE To explore the characteristics of CT images in patients with lobar pulmonary infection and pneumonia.METHODS 69 patients with lobar pulmonary infection and 54 patients with pneumonia-type lung cancer treated in the hospital from Jun.2013 to Jun.2017 were enrolled in the study.Patients with lobar pulmonary infection were taken as the experimental group and pneumonia-type lung cancer patients were taken as the control group.The types and composition ratios of pathogenic bacteria in lobar pulmonary infection were detected.CT scans were performed in both groups.The number of lesions and the distribution of single lesions were compared between the two groups.The imaging features of the two groups (bronchial congestion,nodule shadow,tumor display,and ground-glass surround)were examined and compared.RESULTS In 69 patients,75 strains of pathogens were isolated,of which 24strains of gram-positive bacteria accounted for 32.0%,49 strains of gram-negative bacteria accounted for 65.3%,and 2strains of other strains accounted for 2.7%.In the experimental group,59cases of single lesions accounted for 85.5%,and 10 cases of multiple lesions accounted for 14.5%.In the control group,45 cases of single lesions accounted for 83.3% and 9 cases of multiple lesions accounted for 16.7%.There was no significant difference in the number of lesions between the two groups.There were significant differences in CT findings between the two groups in terms of bronchial congestion,nodule hyperplasia,tumor display,grinding glass shadow,and cystic transillumination (P<0.05).CONCLUSION There was no difference in the number and distribution of lobar pulmonary infections and pneumonia-type lung cancer lesions.CT images showed differences in bronchial congestion,nodule shadows,tumor display,ground glass shadow and cystic transillumination.Attention should be paid to the identification of the above CT image features during the examination.
