摘　　要：目的:分析白细胞免疫球蛋白样受体A3(LILRA3)在类风湿关节炎(RA)患者外周血CD14+单核细胞中的表达。方法:收集2017年2至8月在浙江大学医学院附属第二医院风湿科住院治疗的RA患者53例和21名该院健康体检者的资料。流式细胞术检测CD14^+单核细胞亚群LILRA3表达,并分析其与RA患者的临床特征、实验室指标、自身抗体和疾病活动度的关系。结果:RA患者外周血CD14+单核细胞中LILRA3表达水平较健康对照组升高(P <0. 01)。LILRA3表达与RA患者压痛关节数、肿胀关节数、红细胞沉降率呈正相关(r=0. 280、0. 371、0. 341,P <0. 05或P <0. 01),但与年龄、病程、Sharp评分、C反应蛋白、血常规及免疫球蛋白等指标均无相关性(均P> 0. 05)。类风湿因子和抗环瓜氨酸肽抗体阳性患者LILRA3的表达率高于阴性患者(均P <0. 05)。以28处关节疾病活动指数(DAS28) 5. 1分作为疾病活动度分组的依据,高疾病活动度组LILRA3表达率较中低疾病活动度组升高,差异有统计学意义(P <0. 05)。结论:RA患者外周血CD14+单核细胞中LILRA3表达升高,且与疾病活动度相关。Objective: To investigate the expression of leukocyte immunoglobulinlike receptor A3( LILRA3) in CD14+monocytes of patients with rheumatoid arthritis( RA). Methods: Fifty three RA patients admitted in the Second Affiliated Hospital of Zhejiang University School of Medicine from February 2017 to August 2017,and 21 healthy subjects were enrolled in the study. The expression of LILRA3 in CD14+monocyte subset was determined by flow cytometry,and its correlations with clinical features,laboratory examination results,antibodies and disease activity were analyzed.Results: LILRA3 percentage in the CD14+monocyte subset of RA patients was higher than that in the healthy controls( P < 0. 01). The percentage of LILRA3 was positively correlated with number of tenderness joints,number of swollen joints and erythrocyte sedimentation rate( r = 0. 280,0. 371,0. 341,P < 0. 05 or < 0. 01),but was not correlated with the age,course of disease,Sharp score,C reactive protein,blood routine index and immunoglobulin( all P > 0. 05). In addition,the percentages of LILRA3 in the monocytes of rheumatoid factor( RF)-positive or anti-cyclic citrullinated peptide( CCP) antibody-positive patients were significantly higher than those of the RFor anti-CCP antibody-negative patients( all P < 0. 05); and the percentage of LILRA3 in patients with DAS28 > 5. 1 was higher than that in patients with DAS28≤5. 1( P <0. 05). Conclusion: The expression of LILRA3 is up-regulated in CD14+monocyte subset isolated from RA patients,and it is correlated with disease activity.
