摘　　要：目的 探讨肩关节镜辅助下经皮空心螺钉固定治疗IdebergⅢ型肩胛盂骨折的临床疗效.方法 采用回顾性病例系列研究分析2014年1月-2017年1月西安交通大学医学院附属红会医院收治的28例Ideberg Ⅲ型肩胛盂骨折患者临床资料,其中男19例,女9例;年龄20~49岁[(32.5±3.5)岁].均为Ideberg Ⅲ型肩胛盂骨折.均采用关节镜辅助下经皮空心螺钉固定治疗.记录手术切口长度、术后并发症情况及肩关节活动度.手术前后采用美国加州大学肩关节(UCLA)评分及视觉模拟评分(VAS)评价肩关节功能.结果 28例患者均获随访24 ~36个月[(25.6±4.5)个月].手术切口长度2.0 ~3.7 cm[(2.3±0.8)cm].无感染、骨折块再次移位、不愈合情况.术后1年随访肩关节前屈(160.7±5.5)°,外展(150.8±6.8)°,外旋(38.4±7.5)°.UCLA评分由术前(7.2±1.2)分提高至术后(33.5±3.2)分(P<0.05).VAS由术前(7.8±3.2)分降低至术后(1.3±0.5)分(P<0.05).结论 关节镜辅助下经皮空心螺钉固定微创治疗IdebergⅢ型肩胛盂骨折,患者术后疼痛减轻,并发症少,有利于恢复肩关节功能.Objective To investigate the clinical effect of arthroscopy-assisted percutaneous cannulated screw fixation for Ideberg Ⅲ glenoid fractures.Methods A retrospective case series study was conducted to analyze the clinical data of 28 patients with Ideberg Ⅲ glenoid fracture admitted to the Honghui Hospital Mfiliated to Medical College of Xi'an Jiaotong University from January 2014 to January 2017.There were 19 males and nine females,aged 20-49 years [(32.5 ± 3.5)years].All patients were classified as Ideberg Ⅲ glenoid fractures.All patients were treated with arthroscopy-assisted percutaneous cannulated screw fixation.The length of incision,complications and range of motion of shoulder joint were recorded.The shoulder function was evaluated by University of California at Los Angeles (UCLA) score and visual analogue score (VAS) before and after operation.Results All patients were followed up for 24-36 months [(25.6-± 4.5) months].The incision ranged from 2.0 cm to 3.7 cm [(2.3 ± 0.8) cm].No infection,re-displacement or nonunion occurred.One year after operation,the results of range of motion were as follows:anterior flexion (160.7 ± 5.5)°,abduction (150.8 ± 6.8)°,and external rotation (38.4 ± 7.5) °.The mean UCLA score was increased from preoperative (7.2 ± 1.2) points to (33.5 ± 3.2) points (P < 0.05),and the mean VAS score was decreased from preoperative (7.8 ± 3.2) points to (1.3 ± 0.5) points (P < 0.05).Conclusion For Ideberg Ⅲ glenoid fractures arthroscopyassisted percutaneous cannulated screw fixation can relieve the pain and reduce complications,which is beneficial to the recovery of shoulder function.
