TST联合保留肛垫整形术与外剥内扎术比较

摘　　要：为观察选择性痔上黏膜切除术(TST)联合保留肛垫整形术与外剥内扎术治疗混合痔的疗效,将100例混合痔患者随机分为治疗组和对照组,每组各50例。治疗组采用TST联合保留肛垫整形术治疗,对照组采用外剥内扎术治疗,分别对2组患者临床治愈率,手术时间、住院时间、住院费用,创面愈合时间以及术中出血量、术后疼痛及肛管狭窄等术后并发症指标进行对比观察。结果显示,2组患者在临床治愈率、尿潴留、肛门坠胀发生率方面比较差异无统计学意义(P>0.05)。治疗组患者在手术时间、住院时间、创面愈合时间、术中出血量、术后疼痛及肛管狭窄方面均优于对照组(P <0.05)。但在住院费用方面对照组低于治疗组(P <0.05)。结果表明,TST联合保留肛垫整形术治疗混合痔可提高治疗彻底性,同时更好地保护肛垫及其齿状线区的功能,减少术后并发症,保证肛门精细控便能力不受影响的一种有效治疗混合痔的方法。To observe and compare the effects of TST plus anoeushion-preserving plasty,or external dissection & internal ligation in the treatment of mixed hemorrhoids the 100patients with mixed hemorrhoids were randomly divided into treatment group (n =50,adopted TST plus anoeushion-preserving & plasty) and control one(n =50,external dissection & internal ligation),then both groups curative rate,operative time,hospital stay,hospitalization cost,wound-surface healing time,intraoperative bleeding volume,postoperative pain and stenosis of anus,postoperative complication,etc"indexes were compared.As results,in curative rate,uroschesis and tenesmus of anus there was no statistical difference(P >0.05);but in hospitalization cost treatment group was higher than control one(P<0.05),in other above indexes treatment group was all superior to control one(P<0.05).Results show that TST plus anocushion-preserving & plasty can achieve more throughout treatment effect,more better protect anocushion and dentate line function,reduce postoperative complication,avoid anal accurate stool-controlled capacity to be influenced,thus it is an effective therapy for mixed hemorrhoids.
