摘  要:目的:本课题拟在原有研究的基础上进一步探讨青藤碱对痛风性关节炎(GA)大鼠模型滑膜组织IL-6影响变化,探讨青藤碱治疗GA的可能机制,为临床应用青藤碱防治GA寻找和奠定理论基础。方法:选用SD大鼠60只,随机选出10只作为正常组,命名为Ⅰ组,其余50只采用尿酸钠诱导大鼠痛风性关节炎模型,于造模7d后筛选出造模成功(关节炎指数大于4分)的大鼠共40只,随机等分为4组,每组10只,分别命名为Ⅱ~Ⅴ组。Ⅱ组:双氯芬酸钾组;Ⅲ组:造模+低剂量青藤碱治疗组;Ⅳ组:造模+中剂量青藤碱治疗组;Ⅴ组:造模+高剂量青藤碱治疗组。分别给予Ⅲ组、Ⅳ组和Ⅴ组大鼠后肢大腿上1/3外侧肌肉注射青藤碱注射液15、30、60mg/(kg·d),Ⅱ组每只大鼠给予双氯芬酸钾25mg/(kg·d)灌胃给药,Ⅰ组每只大鼠给予1mL/(kg·d)生理盐水同法注射。每天1次,连续21d。观察GA大鼠踝关节肿胀程度,用放射免疫分析法测定各组大鼠滑膜悬液中IL-6含量;半定量逆转录-聚合酶链反应(RT—PCR)法检测GA大鼠关节滑膜组织IL-6mRNA的表达。结果:(1)踝关节肿胀度比较:第7天时Ⅱ、Ⅲ、Ⅳ、Ⅴ组大鼠踝关节均出现严重肿胀,且Ⅱ、Ⅲ、Ⅳ、Ⅴ组之间关节肿胀程度差异无统计学意义(P>0.05);Ⅱ、Ⅲ、Ⅳ、Ⅴ组踝关节肿胀程度与Ⅰ组比较差异有统计学意义(P<0.05)。第14天和21天时Ⅱ、Ⅲ、Ⅳ、Ⅴ组踝关节肿胀程度均有不同程度减轻,其中Ⅱ、Ⅲ组比较差异无统计学意义(P>0.05),Ⅱ组与Ⅳ、Ⅴ组比较差异均有统计学意义(P<0.05),Ⅰ组与Ⅱ、Ⅲ、Ⅳ组比较差异有统计学意义(P<0.05),Ⅳ组与Ⅴ组比较差异有统计学意义(P<0.05)。其中第14天时Ⅰ组与Ⅴ组比较差异有统计学意义(P<0.05),第21天时Ⅰ组与Ⅴ组比较差异无统计学意义(P>0.05)。(2)第21天时Ⅱ、Ⅲ、Ⅳ、Ⅴ组滑膜悬液中IL-6、IL-6mRNA、关节液白细胞计数、中性粒细胞百分比含量比较:Ⅱ、Ⅲ组比Objective:Based on the original research,this study will further explore the effects of sinomenine on the IL-6 in synovial tissue of GA rat model,and explore the possible mechanism of sinomenine in the treatment of GA. And find and establish a theoretical basis for the clinical application of sinomenine in the prevention and treatment of GA. Methods:Sixty SD rats were selected and 10 were randomly selected as the normal group,which was named as group Ⅰ. The other 50 rats were induced by sodium urate to the gouty arthritis model. After 7 days of modeling,40 rats with successful modeling(arthritis index greater than 4 points)were randomly divided into 4 groups,10 in each group,named II-V group. GroupⅡ:diclofenac potassium group;groupⅢ:modeling + low dose sinomenine treatment group;group Ⅳ:modeling + medium dose sinomenine treatment group;group Ⅴ:modeling+high dose sinomenine treatment group. Sinomenine injection was inject to rats in group Ⅲ,IV and Ⅴ,at 1/3 outer lateral muscle in the hind leg of the rat,and he sinomenine injection was administered with sinomenine injection at 15,30,60 mg/(kg·d),respectively. Every rat in groupⅡ was given diclofenac potassium 25 mg/(kg·d)was administered by gavage. Every rat in group Ⅰ was given 1 mL/(kg·d)of normal saline inject fellow the above inject way. Once a day for 21 consecutive days. The degree of swelling of the ankle joint of GA rats was observed. The level of IL-6 in the synovial suspension of each group was determined by radioimmunoassay. The reverse transcription-polymerase chain reaction(RT-PCR)method was used to detect the expression of IL-6 mRNA in synovial tissue of GA rats. Results:(1)Comparison of swelling degree of ankle joint:On the 7 th day,the ankle joints of the rats in groupsⅡ,Ⅲ,Ⅳ andⅤ showed severe swelling,and there was no significant difference in the degree of joint swelling between groups Ⅱ,Ⅲ,Ⅳ and Ⅴ(P>0.05). The degree of swelling of ankle joints in group Ⅱ,Ⅲ,Ⅳ and Ⅴ were significantly different from that
