摘  要:目的探讨控制调节低温技术对严重创伤性休克患者的凝血功能、血乳酸及血流速度的影响。方法将收治的32例严重创伤性休克患者作为研究组研究对象,通过临床标准化操作规程,运用控制性调节低温技术对患者进行亚低温控制,并研究观察患者的凝血六项指标、血乳酸及腹主动脉血流速度的变化,进行比对分析。结果 32例患者在控制性调节体温达(35. 00±0. 20)℃时的凝血功能、血乳酸指标与控制性调节体温前比较差异均无统计学意义(P> 0. 05);患者的血流速度指标显著低于体温调节前(P <0. 05)。结论运用控制性调节低温技术对严重创伤性低血容量性休克患者进行处置干预后,体表温度达(35. 00±0. 20)℃时凝血功能、血乳酸均稳定,血流速度降低亦能支持限制性液体复苏,提示此亚低温水平安全可靠。Objective To investigate the changes of coagulation function,blood lactic acid and blood flow velocity in patients with severe traumatic shock by regulating and controlling hypothermia. Methods 32 patients with severe traumatic shock were treated as the study group. The patients were controlled by the technique of controlled hypothermia and the six indexes of coagulation were observed. The changes of blood lactic acid and the blood flow velocity of abdominal aorta were compared and analyzed. Results In the study group,the coagulation function,blood lactate index and control temperature were measured at (35.00 ± 0.20) ℃. There was no significant difference between the two groups (P>0.05) , and the blood flow velocity was significantly lower than those before thermoregulation ( P<0.05) . Conclusion After treatment and intervention of severe trauma hypovolemic shock patients with severe trauma and hypovolemic shock,surface temperature reached( 35.00 ± 0.20) ℃ and blood lactate is stable. Lower blood flow velocity also supports the theory of restricted fluid resuscitation,suggesting the safety and reliability of this mild hypothermia level.
