健脾固肾方治疗中老年糖尿病肾病综合征的疗效及对中医证候评分、SCr、BUN指标的影响

摘　　要：目的:研究分析健脾固肾方治疗中老年糖尿病肾病综合征的疗效及对中医证候评分、血肌酐(SCr)、尿素氮(BUN)指标的影响。方法:选择2010年6月-2015年5月在本院就诊的400例老年糖尿病肾病综合征患者为研究对象,根据随机数表法分为对照组和观察组,各200例。对照组应用常规疗法进行治疗,观察组在对照组治疗基础上应用健脾固肾方治疗。于患者治疗1个疗程后评估临床疗效,并对比治疗前后两组中医证候评分及肾功能指标SCr、BUN改善情况。结果:观察组患者治疗后的治疗总有效率83.50%(167/200),高于对照组的69.00%(138/200),差异有统计学意义(P<0.05);治疗前,两组中医证候评分差异无统计学意义(P>0.05);治疗后,两组中医证候评分均低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组SCr、BUN指标水平比较,差异无统计学意义(P>0.05);治疗后,两组SCr、BUN指标水平均低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05)。结论:在老年糖尿病肾病综合征患者中应用健脾固肾方治疗效果显著,可增强临床治疗效果,加速临床症状缓解,减轻肾功能损害,延缓疾病发展。Objective:To study the effects of Jianpi Gushen Formula on the treatment of middle-aged and elderly patients with diabetic nephrotic syndrome and its influence on TCM syndrome score,serum creatinine(SCr) and blood urea nitrogen(BUN).Methods:A total of 400 cases of elderly patients with diabetic nephrotic syndrome from June 2010 to May 2015 in our hospital were selected as the research subjects.All the patients were divided into control group and observation group according to random number table method,200 cases in each group.The patients in the control group were given conventional therapy for treatment,and the patients in the observation group were given Jianpi Gushen Formula for treatment.After a course of treatment,we assessed the clinical efficacy,the TCM symptom scores and the improvement of renal function indexes SCr and BUN.Results:The total effective rate of the observation group after treatment was 83.50%(167/200),which was higher than that of the control group(69.00%,138/200).The difference was statistically significant(P<0.05).Before treatment,there was no significant difference in TCM symptom scores between the two groups(P>0.05).After treatment,the TCM symptom scores in both groups were lower than those before treatment,and the observation group′s was lower than that of the control group and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in SCr or BUN between the two groups(P>0.05).After treatment,the levels of SCr and BUN in both groups were lower than those before treatment,and the observation group′s was lower than that of the control group(P<0.05).Conclusion:Jianpi Gushen Formula has significant effect on elderly patients with diabetic nephrotic syndrome and can enhance the clinical treatment,accelerate clinical symptoms,reduce renal damage and delay the development of the disease.
