羊膜宫腔置入在宫腔粘连分离术后的临床疗效分析

摘　　要：目的分析评价宫腔粘连分离术后宫腔放置羊膜球囊预防宫腔粘连复发的疗效。方法回顾性分析河南省人民医院2015年1月1日—2018年1月1日收治的60例中/重度宫腔粘连的患者,所有患者均行宫腔镜下宫腔粘连分离术,术后服用补佳乐(戊酸雌二醇片)3个月,根据术后处理方法分成两组,观察组(30例):宫腔粘连分离术后宫腔内放置表面被覆羊膜的Foleys管,对照组(30例):宫腔粘连分离术后宫腔内放置Foleys管,均放置1周后取出,术后3个月行宫腔镜检查评估治疗效果。结果2组治疗结果显示,治疗3个月后,观察组月经恢复率明显高于对照组(P<0.05);观察组子宫内膜厚度大于对照组,有显著的统计学意义(P<0.05);治疗6个月后,两组意愿妊娠率观察组大于对照组,差异有统计学意义(P<0.05)。结论新鲜羊膜可以改善宫腔粘连术后临床症状,有效预防宫腔粘连术后再粘连,提高宫腔粘连术后妊娠率。Objective To analyze and evaluate the efficacy of amniotic membrane balloon placement in uterine cavity to prevent recurrence of uterine adhesions after hysteroscopic adhesions separation. Methods A retrospective analysis was made of 60 patients with moderate to severe uterine adhesions admitted to our hospital from January 1,2015 to January1,2018,All patients underwent hysteroscopic hysteroscopic adhesions separation and took it after operation,They were divided into two groups according to post-operative treatment,Foleys tube covered with amniotic membrane was placed in the uterine cavity of observation group( 30 cases),Foleys tube was placed in the uterine cavity of control group( 30 cases),All patients were removed after 1 weeks,and hysteroscopy was performed 3 months after operation to evaluate the therapeutic effect. Results The results of treatment in the two groups showed that the menstrual recovery rate in the observation group was significantly higher than that in the control group after 3 months( P < 0. 05); the endometrial thickness in the observation group was larger than that in the control group,with significant statistical significance( P <0. 05); after 6 months of treatment,the willingness pregnancy rate in the observation group was larger than that in the control group,with statistical significance,Meaning( P < 0. 05). Conclusion Fresh amniotic membrane can improve the clinical symptoms of uterine adhesions,prevent the recurrence of uterine adhesions and improve the pregnancy rate after uterine adhesions.
