低温等离子射频切除累及前联合的早期声门型喉癌疗效分析

摘　　要：目的探讨低温等离子射频消融术(endoscopic low-temperature radiofrequency ablation,RFA)治疗早期声门型喉癌,肿瘤累及前联合是否影响手术疗效。方法我院于2010年9月～2016年6月收治102例早期声门型喉癌患者进行RFA治疗,其中失访3例、2例因其他原因死亡。将97例患者作为研究对象,其中32例为前联合累及的声门型喉癌患者(研究组),无前联合累及患者65例(对比组),将两组手术疗效进行比较。选取48例患者进行嗓音障碍指数(VHI)评估。结果术后研究组的局部控制率为93.7%,32例患者中有2例(6.3%)患者复发,对比组的局部控制率为98.5%,术后65例患者中有1例(1.5%)患者复发,两组的局部控制率无显著差异;术后并发症发生率分别为9.4%、7.7%,无显著统计学差异。结论低温等离子射频消融术治疗早期声门型喉癌时,其疗效显著,肿瘤累及前联合的患者,其手术疗效较未累及前联合的患者无明显差异。Objective To investigate the effect of endoscopic low-temperature radiofrequency ablation(RFA)in the treatment of early glottic carcinoma. Methods From September 2010 to June 2016,102 patients with early glottic carcinoma were treated with RFA. Among them,3 were lost and 2 died of other causes. 97 patients were included in the study. Among them,32 patients were anterior glottic carcinoma(study group)and 65 patients were non-anterior glottic carcinoma(control group). Voice disturbance index(VHI)was used to evaluate 48 patients. Results The local control rate was 93.7% in the study group,2 cases(6.3%)in the 32 patients had recurrence,98.5% in the control group,1 case(1.5%)in the 65 patients had recurrence,and there was no signifi cant difference in the local control rate between the two groups;the incidence of postoperative complications was 9.4% and 7.7%,respectively. Different. Conclusion Low-temperature plasma radiofrequency ablation for early glottic carcinoma has a significant effect. There is no signifi cant difference in the curative effect between patients with tumor involving anterior syndesmosis and those without anterior syndesmosis.
