自拟润肺健脾益肾汤对小儿难治性哮喘肺功能、免疫因子的影响

摘　　要：目的探讨肺脾肾论治在小儿难治性哮喘治疗中的作用及对肺功能、炎性因子与临床症状积分的影响。方法回顾性分析2015年2月至2016年12月本院收治的88例小儿难治性哮喘患儿临床资料,根据治疗方法不同分为对照组和观察组。对照组患儿采用常规西药治疗,观察组患儿在对照组治疗基础上联合自拟润肺健脾益肾汤治疗。观察两组患儿肺功能、临床症状积分及各细胞因子水平变化情况。结果两组患儿治疗后临床症状积分较治疗前改善,观察组患儿治疗后临床症状积分优于对照组(P <0. 05)。两组患儿治疗后用力肺活量(FVC)、第1秒用力呼气容积(FEV1)及最大呼气峰流速(PEF)水平均较治疗前上升,观察组患儿治疗后FVC、FEV1及PEF水平高于对照组(P <0. 05)。两组患儿治疗后IL-8、免疫球蛋白E(IgE)、骨桥蛋白(OPN)及嗜酸性粒细胞阳离子蛋白(ECP)水平较治疗前下降,IFN-γ和皮质醇(Cor)水平较治疗前上升(P <0. 05)。观察组患儿治疗后IL-8、IgE、OPN及ECP水平低于对照组,IFN-γ和Cor水平高于对照组(P <0. 05)。结论运用肺脾肾论治治疗小儿难治性哮喘疗效确切,可有效改善患儿肺功能,调节患儿免疫因子、OPN和ECP水平。Objective To explore the effect of self-made Runfei Jianpi Yishen Decoctionin in the treatment of refractory asthma in children, and its influence on pulmonary function, inflammatory factors and clinical symptom score. Methods 88 children with refractory asthma admitted to our hospital from February 2015 to December 2016 were retrospectively selected and divided into control group and observation group according to different treatment methods. The control group was treated with routine western medicine, while the observation group was treated with self-made Runfei Jianpi Yishen Decoction on the basis of the control group. The changes of pulmonary function, clinical symptom score and cytokine levels in two groups were observed. Results The scores of clinical symptoms in the two groups were improved after treatment, and those in the observation group were better than those in the control group (P<0.05). After treatment, the levels of forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) in the two groups were higher than those before treatment. The levels of FVC, FEV1 and PEF in the observation group were higher than those in the control group (P<0.05). After treatment, the levels of interleukin (IL)-8, IgE, osteopontin (OPN) and eosinophil cationic protein (ECP) in the two groups were decreased, while the levels of interferon (IFN)-γ and Cor were increased (P<0.05). After treatment, the levels of IL-8, IgE, OPN and ECP in the observation group were lower than those in the control group, and the levels of IFN-γ and Cor in the observation group were higher than those in the control group (P<0.05). Conclusions The self-made Runfei Jianpi Yishen Decoctionin is effective in the treatment of refractory asthma in children. It can effectively improve the pulmonary function and regulate the levels of immune factors, OPN and ECP in children.
