化脓性腱鞘炎的诊断与手术治疗

摘　　要：目的:探讨手部化脓性腱鞘炎早期诊断手术治疗,结合VSD负压持续灌注冲洗引流修复的临床疗效。方法:对32例手部化脓性腱鞘炎早期、晚期不同时期观察切开引流,经过扩创、VSD负压置管持续灌注冲洗引流。结果:切开引流应用VSD负压置管持续灌注冲洗引流治疗化脓性腱鞘炎均得到良好恢复。结论:早期发现及早期诊断,早期切开结合VSD置管持续灌注冲洗引流,可完全彻底清理深部鞘内脓性组织,控制组织进一步液化坏死,保持局部组织新鲜,提高肢体的成活率,术后无需换药,减少患者的痛苦。Objective:To investigate the diagnosis and surgical treatment of suppurative tenosynovitis.Mean-while,observe the clinical effect of VSD (vacuum sealing drainage)continuous perfusion and irrigation.Meth- otis:A total of 32 patients with early or advanced stage suppurative tenosynovitis,underwent the procedure of incision and drainage,debridement,VSD negative pressure continuous perfusion and irrigation.Results:The resul't showed that all the patients recovered well.Conclusion:Early discovery,early diagnosis,early incision and drainage combine with VSD negative pressure catheter persistent perfusion and irrigation for treatment"of suppurative tenosynovitis can thoroughly clear the deep purulent tissue,prevent the tissue from further liquefaction necrosis,maintain local fresh tissue,increase the survival rate of limbs and relieve patient's pain.
