摘 要:目的探讨支撑喉镜下声门暴露困难的相关影响因素。方法选取本院2016年9月~2017年9月收治的接受支撑喉镜手术且可能发生声门暴露困难的79例患者作为研究对象,根据术中声门暴露情况分为两组,其中声门暴露患者32例归为观察组,非声门暴露患者47例归为对比组。分析两组患者的性别、年龄、体重、身高、颈围等资料,并建立资料库。分析以上影响因素对支撑喉镜下声门暴露困难的影响。结果单因素分析显示,观察组的体重指数(BMI)、颈围、甲颏距离(TMD)与对比组比较,差异均有统计学意义(P<0.05)。多因素分析显示,BMI≥25 kg/m2、颈围≥40 cm、TMD<6 cm、身高/TMD≥23均是术中声带暴露困难的影响因素。结论支撑喉镜下声门暴露与患者头颈部的解剖特点有关,术前可参照与预测声门的暴露情况,对手术风险进行评估,为治疗提供有效依据,减少术后声门暴露困难情况。Objective To investigate the related influencing factors of glottic exposure difficulty under self-retaining laryngoscope. Methods All of 79 patients who underwent self-retaining laryngoscopy and may occur the glottic exposure difficulty treated in our hospital from September 2016 to September 2017 were selected as the study subjects. According to the intraoperative glottic exposure, they were divided into the two groups. Among them, 32 patients with glottic exposure were assigned to the observation group, while 47 patients with non-glottic exposure were assigned to the control group. The gender, age, weight, height and neck circumference and other data of the two groups were analyzed and a database was established. The influence of the above influencing factors on the difficulty of glottis exposure under the self-retaining laryngoscope was analyzed. Results Univariate analysis showed that the body mass index(BMI), neck circumference and thyromental distance(TMD) in the observation group were compared with those in the control group, and the differences were statistically significant(P<0.05). Multivariate analysis showed that BMI≥25 kg/m2,neck circumference≥40 cm, TMD<6 cm and height/TMD≥23 were all factors influencing the glottis exposure difficulty during surgery. Conclusion The glottic exposure under the self-retaining laryngoscope is related to the characteristics of the head and neck anatomy in the patients. Surgical risk can be assessed by referring to and predicting glottic exposure before surgery, so as to provide an effective basis for treatment and reduce the difficulty of postoperative glottic exposure.
