摘 要:目的研究探讨阿霉素肾病大鼠肾纤维化的改变及抗纤灵方的干预作用。方法将40只SD大鼠随机分为正常组8只,假手术组8只,模型组24只。采用右侧肾切除加尾静脉2次注射阿霉素3 mg/kg制备肾硬化大鼠模型,将造模成功后大鼠随机分为造模组,抗纤灵组,科素亚组。每组分别给予对应药物灌胃,其中抗纤灵组(丹参、制大黄、黄芪、牛膝、桃仁、仙灵脾组成)给药量30 g/(kg·d),科素亚组给药量33 mg/(kg·d),其余各组均给予生理盐水,每次2 mL,共观察8周。通过观察治疗前后大鼠血清生化指标肾功能,尿蛋白定量水平,转化生长因子-β(TGF-β)、结缔组织生长因子(CTGF)蛋白表达,肾脏组织病理形态的变化研究抗纤灵的干预机制。结果抗纤灵方能显著降低血清肌酐,尿素氮,24 h尿蛋白定量,改善肾功能,差异有统计学意义(P <0.05),在24 h尿蛋白的降低方面,抗纤灵组优于科素亚组(P <0.05);抗纤灵组下调TGF-β和CTGF在肾组织蛋白表达,与对照模型组比较,差异有统计学意义(P <0.05),并且抗纤灵组在TGF-β表达水平改变优于科素亚组;在肾组织病理形态观察提示科素亚和抗纤灵组均能从微观形态学观察到肾纤维化的改善,且抗纤灵组优于科素亚组。结论抗纤灵方通过下调TGF-β和CTGF的表达,使阿霉素肾病大鼠的肾功能,24 h蛋白尿得到改善,使肾脏纤维化病理形态缓解改善,并且抗纤灵方疗效优于科素亚。Objective To study the effect of Kangxianling decoction on renal function and renal fibrosis of adriamycin-induced nephropathic rats.Methods Forty SD rats were randomly divided into normal group (n =8), sham operation group (n=8)and model group (n=24).The rat model of renal sclerosis was established by right nephrectomy and intravenous injection of adriamycin for 3mg/kg.The rats were randomly divided into three groups: model group,Kangxianling group and Losartan Potassium group.Each group was given corresponding drugs by intragastric perfusion,and Kangxianling group (Radix Salviae Miltiorrhizae,rhubarb,Radix Astragali,Achyranthes, Peach Kernel and epimedium)was given a dose of 30 g/(kg·d),Losartan Potassium group for 33mg/(kg·d).All other groups were given normal saline for 8 weeks each time for 2mL.Renal function,urine protein,transforming growth factor-β(TGF-β)and connective tissue growth factor (CTGF)were measured before and after treatment. Study on the intervention mechanism of Kangxianling in renal histopathology.Results Kangxianling decoction could significantly reduce serum creatinine,urea nitrogen,24h urine protein quantity and improve renal function (P <0.05). In 24 hours urine protein reduction,Kangxianling group was better than that of Losartan Potassium group (P<0.05). The expression of TGF-β and CTGF in renal tissue in Kangxianling group was significantly lower than that in control group (P<0.05),and the expression level of TGF-β in Kangxianling group was better than that in Losartan Potassium group.Observation of renal histopathology showed that the improvement of renal fibrosis could be observed in both Losartan Potassium and Kangxianling groups,and the improvement of renal fibrosis was better in Kangxianling group than that in Losartan Potassium group.Conclusion By down-regulating the expression of TGF-β and CTGF, Kangxianling prescription improved the renal function and 24h proteinuria of adriamycin nephropathy rats,improved the pathological morphology of renal fibrosis,and the curative e
