摘 要:目的:探讨乳腺断层融合摄影消除了钼靶影像二维重叠影像,提高乳腺病变检出率、降低召回率。方法:统计本院2016年10月~2018年6月乳腺手术患者96例,均先乳腺钼靶摄片,发现可疑病灶,加摄患侧、单体的断层融合摄影。图像由两名主治以上的医生对FFDM、TOMO影像进行分析,参照2013版BI-RADS标准进行分类,并做出综合性的影像诊断。用McNemar检验精确Sig(两两交叉)进行统计学分析。结果:通过对96例乳腺钼靶联合断层融合摄影影像学诊断BIRADS-Ⅳa以上71例。统计学分析,CC+MLO位与断层融合图像两两比较,P=0.00 <0.01,有统计学意义。术后病理结果分别为乳腺癌70例,结合病理结果影像诊断率乳腺癌90.5%。结论:乳腺钼靶与断层融合联合摄影可显示致密性乳腺内部结构及明确腺体结构扭曲原因,提高了病变检出率及降低召回复检率。断层融合摄影作为乳腺检查的新技术,具用良好的应用前景。Purpose: To discuss the removal of two-dimensional overlapping of molybdenum target images by mammography, to improve the detection rate of breast lesions and reduce the recall rate. Methods: Ninety-six cases of breast surgery patients in our hospital from October 2016 to June 2018 were enrolled. All of them were first taken mammography, when suspected lesions were found, the single position tomography of the affected side was taken. The FFDM and TOMO images were analyzed by two or more doctors, and classified according to the 2013 BI-RADS standard, and a comprehensive image diagnosis was made. The McNemar test was used to test the accurate SIG (2 - 2 crosses ) for statistical analysis. Results: Of 96 cases, 71 cases were diagnosed as BI-RADS Ⅳ by mammography combined with tomography. Statistical analysis results showed that there was statistical significant difference between that with CC plus MLO position and with the tomographic fusion image, P<0.01. The post-operative pathological results showed 70 cases of breast cancer, and the imaging diagnosis rate of breast cancer was 90.5 %. Conclusion: Mammography combined with tomographic fusion can be used to display the dense breast internal structure and clarify the cause of gland structure distortion, which can improve the detection rate of lesions and reduce the recall rate. As a new technique for breast examination, tomographic fusion photography has a good application prospect.
