摘　　要：目的:研究子宫肌瘤MRI对比增强特点与高强度聚焦超声(HIFU)消融效果的关系。方法:84例经HIFU治疗的子宫肌瘤患者,治疗前MRI对比增强扫描,按强化程度分为A和B组,按是否均匀强化分为C和D组,分别比较其治疗参数、消融效率和疗效等相关指标,同时评价HIFU治疗的安全性。结果:各组患者的基线数据、治疗参数指标差异无统计学意义(P> 0.05)。A、B两组中,6个月肌瘤减少率差异无统计学意义(P=0.227),其余各指标差异有统计学意义(P <0.05),且均为A组优于B组;C和D组中,6个月肌瘤减少率差异有统计学意义(P=0.029),其余各指标差异无统计学意义(P> 0.05)。术中、术后未观察到严重并发症。结论:MRI对比增强强化程度低的子宫肌瘤,HIFU消融效率、短期疗效更佳。Purpose: To study the relationship between the contrast-enhanced MRI characteristics of uterine fibroids and the ablation effect of high-intensity focused ultrasound (HIFU). Methods: Eighty-four cases of uterine fibroids which were treated with HIFU were enrolled. They were undergone MRI enhancement before treatment. According to the degree of enhancement, these cases were divided into A and B groups; and according to the homogeneity of the enhancement, these cases were divided into C and D groups, respectively. The treatment parameters, ablation efficiency and the treatment effect were compared between different groups. The safety of HIFU treatment was also evaluated. Results: There were no statistically significant differences in baseline data and treatment parameters between each group (P>0.05). There was no significant difference in the reduction rate of fibroids between the A and B groups at 6 months (P=0.227), and the difference of remaining indexes were with statistical significant (P<0.05), and those of Group A were all superior to those of Group B. The difference of reduction rate of fibroids at 6 months was with statistical significant between Group C and D (P=0.029). There was no statistical significant difference of the remaining indicators (P>0.05). No serious complication was observed. Conclusion: The efficacy of HIFU ablation for uterine fibroids with low enhancement on contrast-enhanced MRI is better.
