摘　　要：陈小砖教授认为胸椎小关节紊乱病位在胸背,其病机不外虚实两端,虚证属气血阴阳亏虚,局部经脉筋膜失于濡养,实证乃痰浊、湿邪、瘀血、气滞等为患,而致胸背部气血受阻,血行不畅,痰瘀为病是重要的致病因素。陈小砖教授诊断上强调"三步定位"诊断法及胸椎棘突触诊的重要性,治疗上强调手法为先、上下同治、筋骨并重、针药结合,并擅于从痰瘀论治。陈教授临床不拘一格,圆机活法,根据不同的病情和疾病的不同阶段选择针刺、刺血、中药内服、药膏外敷、拔罐等法。陈教授总结探索出精确的零角度胸椎微调法用于胸椎小关节的复位,定位准确,疗效立竿见影。Professor Chen Xiao-zhuan believes that the disease of thoracic facet joint disorder lies in the chest and back,and its pathogenesis includes both deficiency and excess. Deficiency syndrome means deficiency of qi and blood,yin and yang,which leads to loss of nourishment of local meridian fascia. Excess syndrome means phlegm turbidity,dampness evil,blood stasis and qi stagnation,which results in obstruction of qi and blood in the chest and back,due to obstruction of blood circulation,phlegm and blood stasis. Professor Chen Xiao-zhuan emphasizes the importance of " three-step positioning" diagnosis" and palpation of thoracic spinous process. In terms of treatment,he emphasizes that manipulation should be in the first place. He pays attention to both upper and lower treatment,both muscles and bones,combination of acupuncture and medicine. He is good at treating phlegm and blood stasis. Professor Chen is not rigid in clinical practice. He chooses acupuncture,pricking blood,taking traditional Chinese medicine orally,applying ointment externally,cupping and so on according to different conditions and different stages of disease.Professor Chen has summarized and explored a precise zero-angle thoracic spine fine-tuning method for the reduction of thoracic facet joints with accurate positioning and immediate effect.
