温胞饮加减治疗肾阳虚型月经后期的临床观察

摘　　要：目的:观察中药温胞饮加减治疗肾阳虚型月经后期的疗效及对卵泡发育、排卵的影响。方法:将符合纳入标准的患者60例,随机分为观察组和对照组,每组30例,观察组予温胞饮加减内服,对照组予妇科再造胶囊内服,服药三个月经周期后观察两组患者的疗效、排卵情况及孕酮值的变化。结果:1.中医症候改善:观察组有效率86. 7%,对照组有效率80%,两组比较无差异(P>0. 05); 2.中医症候积分变化:两组患者在治疗后中医症候积分均较治疗前明显下降,组内前后比较差异有统计学意义(P <0. 01),观察组中医症候积分下降较对照组明显,两组间比较差异有统计学意义(P<0. 05); 3.排卵率:观察组60%,对照组33. 3%,两组比较有显著性差异(P<0. 05); 4.黄体期血清孕酮值:观察组:(52. 94±21. 384) nmol/L,对照组:(37. 30±12. 356) nmol/L,两组比较有显著性差异(P<0. 05)。结论:温胞饮加减及妇科再造胶囊治疗肾阳虚型月经后期,均可有效改善月经周期及相关症侯,但前者对促进卵泡发育、排卵及改善黄体功能作用更为明显。Objective: To investigate the effect of a traditional Chinese medicine( Wen Baoyin) for treatment of delayed menorrhea caused by follicular development and ovulation delayed owing to Kidney-Yang-Deficient. Methods: 60 patients of such condition were randomly divided into experimental( Wen Baoyin) and control( oral gynecological reconstruction capsule) group with 30 patients in each group. After the treatment for three months,the curative effects,ovulation and the change of progesterone of patients were investigated. Results: 1. The improvement of curative effect of TCM syndrome: the effective rate of the experimental group was86.7% while the control group was 80%. There was no significant difference between two groups( P>0. 05); 2.Changes of TCM syndrome score: the symptom scores of patients decreased obviously for two groups after treatment with Wen Baoyin,group difference was statistically significant( P<0. 01),the observation group of TCM syndrome integral decreased significantly compared with the control group,there was significant differences between two groups( P<0. 05); 3. The ovulation rate is 60% for experimental group and 33. 3% for the control group. Obviously,two groups are significantly different from each other in this aspect.( P<0. 05); 4. The serum progesterone level in luteal stage: the experimental group was 52. 94±21. 384 nmol/L,while the control group was 37. 30 ± 12. 356 nmol/L,thus this value is statistically significant difference for two groups( P < 0. 05);Conclusions: The effect of both Wenbaoyin Decoction treatment and oral gynecological reconstruction capsule is significant for treatment of delayed menorrhea which caused by follicular development and ovulation delayed owing to Kidney-Yang-Deficient. Both of them can effectively improve the menstrual cycle and the related diseases,but the former significantly promote the follicular development,ovulation and luteal function.
