摘 要:目的探讨髌外侧支持带松解术治疗髌骨外侧高压综合征(excessive lateral pressure syndrome,ELPS)的效果。方法选取2016年1月至2017年3月在我院治疗的ELPS患者87例,所有患者均行髌外侧支持带松解术治疗,观察术前及术后12个月膝关节Lysholm评分、影像学指标、等速肌力测试结果及步态参数。结果 Lysholm评分:术前(56.61±6.84)分,术后12个月(83.22±7.10)分(P<0.05);髌股适合角(CA):术前(23.54±4.22)°,术后12个月(14.16±3.13)°(P<0.05);髌骨倾斜角(PTA):术前(12.16±1.14)°,术后12个月(10.84±1.17)°(P<0.05);髌骨外移度(PLS):术前(12.30±1.87) mm,术后12个月(10.84±1.71) mm (P<0.05);在角速度60°/s,膝关节伸肌:术前(57.22±12.46) N·m,术后12个月(76.41±20.22) N·m (P<0.05);屈肌峰力矩:术前(37.15±9.57) N·m,术后12个月(46.54±8.14) N·m(P<0.05);步长:术前(41.32±3.64) cm,术后12个月(50.44±2.97) cm (P<0.05);步幅:术前(80.16±8.10) cm,术后6个月(97.76±7.11) cm,步速:术前(58.24±4.16) cm/s,术后12个月(78.44±5.20) cm/s(P<0.05)。结论髌外侧支持带松解术治疗ELPS有较好的临床效果,值得临床应用。Objective To investigate effects of lateral patellar retinaculum release in the treatment of excessive lateral pressure syndrome (ELPS). Methods From January 2016 to March 2017, 87 patients with ELPS were selected in our hospital. All patients were treated with lateral patellar retinaculum release. Lysholm score, imaging index, isokinetic muscle strength test results and gait parameters were observed before and 6 months after the operation. Results Lysholm score: (56.61±6.84)preoperatively, (83.22±7.10)12 months postoperatively (P<0.05); congruence angle (CA): (23.54±4.22)° preoperatively, (14.16±3.13)° 12 months postoperatively (P<0.05); patellar tilt angle (PTA): (12.16±1.14)° preoperatively, (10.84±1.17)° 12 months postoperatively (P<0.05); patellar lateral shift (PLS): (12.30±1.87)mm preoperatively, (10.84±1.71)mm 12 months postoperatively (P<0.05). At the angular velocity of 60°/s, the extensor of knee joint: (57.22±12.46)N · m preoperatively, (76.41±20.22)N · m 12 months postoperatively (P<0.05); flexor peak torque: (37.15±9.57)N · m preoperatively, (46.54±8.14)N · m 12 months postoperatively (P<0.05); step length: (41.32±3.64)cm preoperatively, (50.44±2.97)cm 12 months postoperatively (P<0.05); stride: (80.16±8.10)cm preoperatively, (97.76±7.11)cm 12 months postoperatively; walking speed: (58.24±4.16)cm/s preoperatively, (78.44±5.20)cm/s 12 months postoperatively (P<0.05). Conclusions Lateral patellar retinaculum release has good clinical effects in the treatment of ELPS, and it is worthy of clinical application.
