济南市接受HIV非职业暴露预防男性就诊者高危行为及阻断效果研究

摘　　要：目的了解济南市传染病医院接受HIV非职业暴露后预防(nPEP)的男性就诊者高危行为及阻断效果情况,为尽快制定统一合理的nPEP指南提供参考。方法于2016年10月至2017年12月,搜集济南市传染病医院性病门诊就诊者中接受nPEP的男性,调查他们的一般情况、高危行为状况、边缘性行为情况等。调查前和接受nPEP服务后两周、四周、三个月分别采集血样并进行HIV抗体检测。结果共计调查接受nPEP的男性171人,平均年龄为(32.2±8.6)岁;39人(占22.8%)户籍地为济南市,39人(占22.8%)就诊时明确知道对方是HIV感染者或病人;132人(占77.2%)就诊时知道对方是男男性行为者(MSM),87人(占50.9%)有未使用安全套被动接受肛交行为;40人(占23.4%)有未使用安全套插入阴道行为。接受nPEP后进行HIV检测,未发现转阳情况。结论济南市接受nPEP男性就诊者多数为MSM,且多数发生过无保护性行为,应尽快制定统一合理的nPEP服务指南以便阻断更多的艾滋病感染。Objective To understand the occurrence of high-risk behavior among men receiving nPEP in Jinan,and to provide a reference for development of nPEP guideline.Methods From Oct 2016 to Decr 2017,men receiving nPEP were recruited from sexually transmitted diseases clinics at Jinan infectious disease hospital to investigate their general conditions,high-risk behaviors,and marginal behavior.Blood samples were collected and HIV antibody tests were conducted before the investigation and three months after receiving nPEP services.Results A total of 171 men were surveyed for nPEP,with an average age of 32.2±8.6 years old.39 people(22.8%)were Jinan residents and clearly knew that their sexual partners were HIV-infected.132 people(77.2%)knew that their sexual partners were men having sex with men.87 people(50.9%)did not use condoms while having anal sex.40 people(23.4%)did not use condom while having vaginal intercourse.No HIV conversion was found due to nPEP.Conclusion In Jinan,most men receiving nPEP are MSM,who have unprotected sexual behavior.We should develop a guide for expansion of nPEP and blocking HIV spreading.
