摘　　要：目的探索男男性行为者(MSM)中新发现的艾滋病病毒(HIV)感染者的首次CD4^+T淋巴细胞(简称CD4细胞)检测情况及其影响因素,为制定有针对性的艾滋病(AIDS)防治措施提供依据。方法采用自行设计的问卷,于2015年1月至2017年8月,对上海市静安区MSM中新发现的HIV感染者进行调查,内容包括人口学特征、AIDS知识知晓情况、行为学特征、身体状况等信息,并随访其首次CD4细胞检测情况。结果共有207例MSM中新发现的HIV感染者纳入研究,首次CD4细胞检测及时率为53.1%(110例),计数均值为(342.0±183.2)个/μL,其中≤350个/μL者占52.7%(109例)。多因素Logistic回归分析显示:高中及以上文化程度[比值比(OR)=3.90,95%可信区间(CI):1.41~10.83]、有固定性伴者(OR=2.20,95%CI:1.18~4.11)、能自理者(OR=3.05,95%CI:1.02~9.15)、样本来源于自愿咨询检测门诊(VCT)(OR=1.83,95%CI:1.02~3.25)的感染者,更倾向于及时进行首次CD4细胞检测。结论静安区MSM中新发现的HIV感染者首次CD4细胞检测不及时率和诊断延迟比例相对较高,应制定针对性措施提高检测及时率,做到"早发现、早管理、早治疗",进而预防HIV在MSM中的传播。Objective To analyze the first CD4+T lymphocyte count of newly detected HIV-positive men who have sex with men(MSM)in Jing’an district,and to provide scientific evidence for HIV/AIDS prevention and control.Methods A self-designed questionnaire survey was conducted in the newly reported MSM infected with HIV from Jan 2015 to Aug 2017.Information of demographic and behavior characteristics,awareness of AIDS knowledge,and current physical condition etc.were collected,followed by the first CD4 cell testing.Results 207 HIVpositive MSM participated in the study,with 53.1%(110/207)receiving the first CD4 cell testing,and the average value of the first CD4 cell was 342.0±183.2 cell/uL,with 52.7%(109/207)lower than 350 cell/μL.Multivariate logistic regression analysis showed that the HIV positive MSM with education level of high school and above(OR=3.90,95% CI:1.41-10.83),fixed sexual partners(OR=2.20,95% CI:1.18-4.11),self-care ability(OR=3.05,95% CI:1.02-9.15),and visits to VCT clinics(OR=1.83,95%CI:1.02-3.25),would have timely test of the first CD4 cell.Conclusion The newly detected HIV-positive MSM in Jing’an district have a lower rate of timely first CD4 cell testing and higher rate of delayed diagnosis.We should take measures to enhance the timely CD4 cell testing,and assure "early detection,early management and early treatment",so as to reduce HIV spreading among MSM.
