RPH与PPH在重度混合痔治疗中的疗效与安全性对比评价

摘　　要：目的比较痔疮自动套扎术(RPH)与吻合器痔上黏膜环形切除术(PPH)在重度混合痔治疗中的疗效与安全性。方法 120例重度混合痔患者,采用随机数字表法分为试验组和对照组,每组60例。试验组患者采用RPH治疗,对照组患者采用PPH治疗。对比两组患者手术情况、并发症发生情况。结果试验组患者手术时间(14.26±2.05)min、下床活动时间(14.25±3.05)h、住院时间(5.02±0.26)d均短于对照组的(20.86±3.28)min、(19.98±4.28)h、(7.38±0.41)d,术中出血量(5.26±0.42)ml少于对照组的(10.44±0.61)ml,差异具有统计学意义(P<0.05)。试验组患者并发症发生率显著低于对照组,差异具有统计学意义(χ~2=6.9818, P=0.0082<0.05)。结论 RPH手术可有效缩短重度混合痔患者治疗时间,减少并发症发生,效果显著,值得借鉴。Objective To compare the efficacy and safety of Rubber procedure of hemorrhoids (RPH) and procedure for prolapse and hemorrhoids (PPH) in the treatment of severe mixed hemorrhoids. Methods A total of 120 patients with severe mixed hemorrhoids were divided by random number table into experimental group and control group, with 60 cases in each group. The experimental group was treated with RPH, and the control group was treated with PPH. The operation status and occurrence of complications between the two groups were compared. Results The experimental group had shorter operation time as (14.26±2.05) min, off-bed activity time as (14.25±3.05) h and hospitalization time as (5.02±0.26) d than (20.86±3.28) min, (19.98±4.28) h and (7.38±0.41) d in the control group, and less intraoperative bleeding volume as (5.26±0.42) ml than (10.44± 0.61) ml in the control group. The differences were statistically significant (P<0.05). The experimental group had significantly lower incidence of complications than the control group, and the difference was statistically significant (χ2=6.9818, P=0.0082<0.05). Conclusion RPH shows remarkable effect, and can effectively shorten the treatment time of severe mixed hemorrhoids patients and reduce complications. It is worth reference.
