克罗恩病患者居家肠内营养护理干预及其效果研究

摘　　要：目的评价居家营养在克罗恩病患者家庭肠内营养中的实施效果。方法回顾性分析在南京总医院临床营养治疗中心登记在册、行家庭肠内营养支持的CD患者156例,依托营养支持多学科团队,开展家庭营养信息访视,重点围绕营养教育、营养监测及并发症处理、随访制度建立等对居家克罗恩病患者行肠内营养支持进行管理;在出院前、出院后1个月、2个月、3个月进行间接能量测定和体质分析及生存质量评分(SF-36),以评价居家肠内营养支持的效果。纳入数据采用SPSS 21.0进行分析,计数资料以例数和百分比描述,计量资料以均数±标准差(x±s)表示,各组间采取方差分析。结果接受家庭营养支持的156例CD患者营养状况较出院时明显改善(P<0.05),生活质量水平较前显著提高(P<0.05)。结论居家营养支持可显著改善患者的营养状况,提高患者的生活质量,具有可行性,值得推广应用,但规范化的肠内营养支持护理管理流程及质量体系的建立还有待于进一步研究。Objective To evaluate of home nutrition in the implementation of enteral nutrition in Crohn’s disease patients.Methods A retrospective analysis of 156 patients with CD enrolled in registration for home enteral nutrition clinical nutrition treatment center of Nanjing general hospital,relying on the nutritional support of the multidisciplinary team,carry out family nutrition information visiting platform,focusing on nutrition education,nutrition monitoring and treatment of complications,follow-up system was established to manage the home in patients with Crohn’s disease enteral nutrition support; in the hospital before and after 1 months and 2 months,3 months,for indirect determination of energy and physical analysis and the quality of life score( SF-36),to evaluate the effect of home enteral nutrition support.Results The nutritional status of 156 patients with CD who received home nutrition support was significantly improved( P<0.05),and also the quality of life( P<0.05).Conclusion Home nutrition support can improve the nutritional status and quality of life of patients,it is feasible and worthy of popularization.The establishment of standardized enteral nutrition support nursing management process and quality system remains to be studied further.
