经直肠彩色多普勒超声结合宫腔超声造影技术诊断子宫内膜息肉可行性

摘　　要：目的:评估经直肠多普勒超声扫查结合宫腔超声造影技术(TRS+SHG)对子宫内膜息肉的诊断意义。方法:收集149例经腹部和阴道超声(TVS)扫查诊断疑似子宫内膜息肉的患者资料,均行TRS+SHG后再行宫腔镜(HS)治疗及病理诊断。结果:149例TVS诊断疑似子宫内膜息肉患者取组织病检,确诊110例,TRS+SHG诊断符合95例,HS诊断符合97例。TVS,TRS+SHG,HS诊断准确率分别为:73.82%,86.36%,88.18%;误诊率分别为:26. 18%,5.0%,4.9%。TVS与TRS+SHG组、HS组诊断子宫内膜息肉准确率比较,差异均有统计学意义(P<0.05);TRS+SHG、HS组诊断子宫内膜息肉差异无统计学意义(P>0.05)。结论:TRS+SHG在诊断子宫内膜息肉方面明显优于TVS,且简便易行、安全高效,准确率、灵敏度高,与HS结果一致性较好。Objective To investigate the clinical value of transrectal sonography combined with sonohysterography( TRS+SHG) in the diagnosis of endometrial polyps. Methods Data of 149 patients with suspected endometrial polyps diagnosed by abdominal and transvaginal sonography( TVS) were collected. All patients were treated by TRS+SHG followed by hysteroscopy( HS) and pathological diagnosis. Results In 149 cases of suspected endometrial polyps diagnosed by TVS,110 cases were confirmed by histological examination,95 cases by TRS+SHG and 97 cases by HS. The diagnostic accuracy rates of TVS,TRS+SHG and HS were 73.82%,86.36% and 88.18% respectively,and the misdiagnosis rates were 26.18%,5.0%and 4.9% respectively. There were significant differences in the accuracy of diagnosis of endometrial polyps between TVS and TRS+SHG group and HS group( P<0.05),but there was no significant difference in the diagnosis of endometrial polyps between TRS+SHG and HS groups( P>0.05). Conclusion TRS+SHG is superior to TVS in the diagnosis of endometrial polyps. It is simple,safe,efficient,accurate,sensitive,and consistent with HS results.
