冠心病患者血清胆红素水平与左心室肥厚的相关性研究

摘　　要：目的探讨冠心病患者血清胆红素水平与左心室肥厚(LVH)的相关性。方法选取200例冠心病患者,根据左心室质量指数(LVMI)分为左心室肥厚组(73例)和左心室正常组(127例),收集其空腹血样本用全自动生化仪测定血脂、血糖、肝肾功能、血清胆红素水平等,并对所有冠心病患者行心脏超声检查。结果在冠心病患者中,左心室肥厚组LVMI高于左心室正常组,而血清胆红素水平低于左心室正常组,且差异均有统计学意义(P <0. 05); Pearson相关分析提示,在冠心病患者中,LVMI与血清胆红素水平呈负相关(r=-0. 221,P <0. 001); Logistic回归分析提示,血清胆红素水平是冠心病合并LVH的独立影响因素(OR=0. 509,95%CI:0. 314～0. 743,P <0. 05)。结论冠心病合并LVH者血清胆红素水平降低,血清胆红素水平是冠心病合并LVH的保护因素。Objective To explore the relationship of serum bilirubin and left ventricular hypertrophy( LVH) in coronary heart disease. Methods A total of 200 patients with coronary heart disease were divided into LVH group( n = 73) and non-LVH group( n = 127) according to left ventricular myocardial mass index( LVMI). Fasting blood samples were collected for the detections of blood lipids,blood sugar,liver and kidney function,serum bilirubin levels by an automatic biochemical analyzer,and echocardiography was performed on all patients with coronary heart disease. Results LVMI was higher while serum bilirubin level was lower in the LVH group than that in the non-LVH group,with the differences statistically significant( P <0. 05). Pearson correlation analysis showed that LVMI was negatively correlated with serum total bilirubin level( r =-0. 221,P < 0. 001). According to Logistic regression analysis,serum bilirubin level was an independent factor for coronary heart disease combined with LVH( OR = 0. 509,95% CI: 0. 314-0. 743,P < 0. 05). Conclusion Serum bilirubin level decreases in patients with coronary heart disease combined with LVH,which is an independent protective factor for LVH combined with coronary heart disease.
