血清Cys-C联合β2-MG检测对高血压肾病的早期诊断价值

摘　　要：目的研究血清胱抑素(Cys-C)联合β2-微球蛋白(β2-MG)检测对高血压肾病早期的诊断价值。方法随机选取长治市第二人民医院2012年3月至2017年3月收治的80例高血压肾病患者作为研究组,另随机选取80例健康成人作为对照组。采用免疫投射比浊法检测研究组血清Cys-C和β2-MG水平。比较研究组治疗前后血压及心率、各级高血压肾病患者炎性因子的阳性检出率;观察研究组治疗前后炎性因子水平及生活质量评分的变化,并与对照组比较。结果研究组各级患者Cys-C和β2-MG阳性检出率高于尿素氮(UREA)和肌酐(Cr)阳性检出率(P<0.05)。研究组治疗前心率、舒张压和收缩压高于治疗后(P<0.05)。治疗前,研究组Cys-C、β2-MG、UREA和Cr水平高于对照组(P<0.05);治疗后,研究组Cys-C、β2-MG、UREA和Cr水平与对照组比较差异未见统计学意义(P>0.05)。治疗前研究组生活质量评分低于对照组(P<0.05);治疗后,研究组生活质量评分与对照组比较差异未见统计学意义(P>0.05)。结论Cys-C联合β2-MG对高血压肾病早期诊断价值较高,可以有效判断患者高血压肾病的严重程度,为临床对该病的早期诊断和治疗提供强有力的证据,值得在临床上推广应用。Objective To study the significance of Cys-C combined with β2-MG in the early diagnosis of hypertensive nephropathy.Methods Eighty patients with hypertensive nephropathy who were hospitalized in the Second People's Hospital of Changzhi from March 2012 to March 2017 were randomly selected as study group.Another 80 healthy adults were selected as control group.Levels of serum cystatin (Cys-C)and β2-microglobulinthe (β2-MG)of study group were detected by immune serum turbidity. Blood pressure and heart rate (HR)before and after treatment were compared,and positive rates were compared among patients with different degrees of hypertensive nephropathy in study group.Levels of inflammatory factors and scores of quality of life were compared between study group and control group, before and after treatment.Results Positive rates of Cys-C and β2-MG were significantly higher than that of urea and creatinine (Cr)in patients with different degrees of disease in study group (P<0.05).In study group,the heart rate (HR),diastolic blood pressure and systolic blood pressure before treatment were significantly higher than those after treatment (P>0.05).Bofore treatment,the levels of Cys-C,β2-MG,UREA and Cr in study group were significantly highter than those in control group (P<0.05).After treatment,there was no significant difference in levels of Cys-C,β2-MG,urea or Cr between study group and control group (P<0.05).Before treatment,the scores of quality of life in study group were significantly lower than those in control group (P<0.05);after treatment,there was no significant difference in scores of quality of life between study group and control group (P>0.05). Conclusions Cys-C combined with β2-MG has better effect in the early diagnosis of hypertensive nephropathy.It can effectively identify the severity of hypertensive nephropathy and provide strong evidence for the early diagnosis and treatment.So it is worthy of promotion and application in clinical practice.
