高频彩超对乳腺叶状肿瘤和纤维腺瘤的鉴别诊断价值

摘　　要：目的分析高频彩超鉴别诊断乳腺叶状肿瘤(PTB)与纤维腺瘤(BF)的临床价值。方法回顾性分析2016年6月至2017年5月安阳市肿瘤医院治疗的58例PTB患者和60例BF患者的临床资料,58例PTB患者作为PTB组,60例BF患者作为BF组。入选者均接受高频彩超检查,观察PTB组、BF组二维超声特征、血流信号分级等。结果PTB组、BF组边界清晰度比较,差异未见统计学意义(P>0.05);PTB组多可见无回声区、钙化、形态不规则或欠规则、后方回声增强、内部回声不均匀等二维超声特征,BF组则多可见形态规则、后方回声无改变、内部回声均匀或欠均匀等二维超声特征;两组钙化、无回声、形态、后方回声、内部回声等比较,差异有统计学意义(P<0.05);与BF组比较,PTB组血流信号分级相对较高,差异有统计学意义(P<0.05)。结论高频彩超用于BF和PTB的检查有利于提供清晰的超声图像,可通过观察声像图特征、血流特征鉴别诊断BF和PTB,为临床治疗提供确切参考依据。ObjectiveTo analyze the clinical value of high frequency color Doppler ultrasound in the differential diagnosis of breast lobe tumors (PTB) and fibroadenoma (BF).MethodsThe clinical data of 58 PTB patients and 60 BF patients treated in Anyang Tumor Hospital from June 2016 to May 2017 were retrospectively analyzed. The 58 PTB patients as PTB group, 60 BF patients as BF group. All the participants were examined by high-frequency color Doppler ultrasound, and the characteristics of two-dimensional ultrasound and blood flow signal were observed in PTB group and BF group.ResultsThe boundary clarity in PTB group and BF group showed no significant difference (P>0.05). Non-echoic area, calcification, irregular or irregular shape, rear echo enhancement, and inhomogeneous internal echo were mostly visible in the PTB group, while two-dimensional ultrasonic features such as morphological rule, no change in rear echo, and uniform or inhomogeneous internal echo were mostly visible in the BF group. The two groups showed significant differences in calcification, non-echo, morphology, rear echo, and internal echo (P<0.05). Compared with the BF group, the PTB group had relatively higher blood signal grading, and the difference was significant (P<0.05).ConclusionsHigh frequency color doppler ultrasound can be used to provide clear ultrasound images in examination of BF and PTB, and then the diagnosis of BF and PTB can be made by observing the features of sonograms and blood flow, so as to provide accurate reference for clinical treatment.
