Foley尿管低位小水囊与小剂量米索前列醇在孕晚期引产中的作用比较

摘　　要：目的比较Foley尿管低位小水囊与小剂量米索前列醇在孕晚期引产中的应用效果。方法选择2017年1月至2017年12月自愿在新乡市妇幼保健院引产的孕晚期初产妇122例作为研究对象,将其按随机数表法分为两组,每组61例。观察组采取米索前列醇引产,对照组采取Foley尿管小水囊引产。观察两组引产效果、促宫颈成熟效果、分娩结局及不良反应。结果两组引产总有效率、治疗前后宫颈Bishop评分、产后出血量、新生儿Apgar评分及胎膜早破、排尿不畅发生率比较,差异未见统计学意义(P>0.05);与对照组比较,观察组阴道分娩、宫缩过频发生率(77.05%、13.11%)较高,宫颈出血发生率(0.00%)较低,差异有统计学意义(P<0.05)。结论将Foley尿管低位小水囊、小剂量米索前列醇用于孕晚期引产中促宫颈成熟效果好,引产成功率高,其中米索前列醇可减少宫颈出血率及剖宫产率,但可诱发宫缩过频不良反应。ObjectiveTo compare the application effects of Foley ureter low position small water bag and low dose of misoprostol on induced labor during third trimester.MethodsA total of 122 primiparae in third trimester with induced labor from January 2017 to December 2017 in Xinxiang Maternal and Child Health Care Hospital were divided into two groups by random number table, with 61 cases in each group. The patients in the observation group were given induced labor with misoprostol, while the patients in the control group were given induced labor with Foley ureter low position small water bag. The induced labor effect, effect of cervical ripening, delivery outcome, adverse reactions in the two groups were observed.ResultsThere was no significant difference in the total effective rate of induced labor, cervical Bishop score before and after treatment, postpartum bleeding volume, neonatal Apgar score, incidence of premature rupture of membrane or dribble urination between the two groups (P>0.05); compared with the control group, the incidence of vaginal delivery and frequent uterine contraction in observation group (77.05%, 13.11%) were higher, but the incidence of cervical hemorrhage in the observation group (0.00%) was lower (P<0.05).ConclusionsFoley ureter low position small water bag and low dose of misoprostol in induced labor during third trimester have better effects of cervical ripening and high success rate of induced labor. Misoprostol can decrease the rate of cervical hemorrhage and cesarean section, but it will induce adverse reactions such as frequent uterine contraction.
