摘　　要：目的研究地塞米松磷酸钠注射液不同给药途径对突发性聋疗效的影响,探讨鼓室注射地塞米松磷酸钠注射液联合全身用地塞米松磷酸钠注射液治疗突发性聋的疗效是否优于单一途径用地塞米松磷酸钠注射液。方法收集2012年1月～2017年9月在石河子大学医学院第一附属医院耳鼻咽喉科住院治疗的单侧突发性聋患者的临床资料进行分析,共收集78例,根据所接受的地塞米松磷酸钠注射液不同途径分成3个组进行比较。A组:全身应用地塞米松磷酸钠注射液组(22例);B组:鼓室注射地塞米松磷酸钠注射液组(23例);C组:鼓室注射地塞米松磷酸钠注射液+全身应用地塞米松磷酸钠注射液组(33例);比较3个组的治疗效果。结果各组治疗前后听力改善值有统计学意义。A组总有效率59.10%,B组总有效率56.52%,C组总有效率69.70%,C组与另两组分别比较,差异无统计学意义。结论鼓室注射地塞米松磷酸钠注射液治疗突发性聋能达到全身用地塞米松磷酸钠注射液的同样疗效,地塞米松磷酸钠注射液联合用药并不优于单一途径用药。OBJECTIVE To evaluate the therapeutic effects of different administration routes of dexamethasone on sudden hearing loss(SHL).To explore whether the clinical effect of intratympanic injection combined with intravenous dexamethasone is better than single injection ofdexamethasone.METHODS 78 patients with unilateral SHL were enrolled from January 2012 to September 2017 in the Department of Otolaryngology of First Affiliated Hospital of Shihezi University.Patients were divided into three groups by different administration routes of dexamethasone,systemic therapy of dexamethasone group (group A,22 cases),intratympanic injection of dexamethasone group (group B,23 cases)and intratympanic combined with systemic injection of dexamethasone group(group C,33 cases).RESULTS The post-treatment pure tone averages (PTA)in the three groups decreased significantly.Total effective rates of the group A,B and C were 59.10%,56.52%,69.70%,respectively.The PTA of Group C was not significantly different compared with that of group A and group B.CONCLUSION The efficiency of intratympanic injection ofdexamethasone for SHL is similar to that of systemic application of dexamethasone .The efficacy of intratympanic combined with systemic injection of dexamethasone in the treatment of sudden hearing loss is not better than the single injection of dexamethasone.
