摘 要:目的分析外周血中血小板(PLT)和CD3^+、CD4^+、CD8^+淋巴细胞水平与乳腺癌临床病理特征的相关性。方法将2016年1月至2018年3月绍兴市中医院收治的85例女性原发浸润性乳腺癌患者作为病例组,另同期选取30例体检健康女性志愿者为对照组。测定两组患者外周血PLT和CD3^+、CD4^+、CD8^+淋巴细胞的水平并进行比较,分析外周血PLT和CD3^+、CD4^+、CD8^+淋巴细胞与乳腺癌临床病理特征的关系,并采用Pearson相关性分析乳腺癌患者外周血PLT与CD3^+、CD4^+、CD8^+淋巴细胞水平的关系。结果病例组外周血PLT[(247.80±66.85)×109/L]和CD8^+[(30.85±4.99)%]的水平较对照组均显著上升,CD3^+[(56.70±6.93)%]、CD4^+[(24.64±4.84)%]的水平较对照组[PLT:(118.97±16.35)×109/L、CD8^+:(23.05±3.14)%、CD3^+:(65.77±7.35)%、CD4^+:(31.90±3.25)%]均显著下降(t值分别为7.37、6.07、7.62、8.01,均P<0.01)。病例组患者不同年龄、病理类型及肿瘤大小之间PLT和CD4^+水平的比较,均无明显差异(t值分别为0.60、0.22、0.36、0.51、0.39、0.27,均P>0.05)。伴有淋巴结转移的患者外周血PLT水平较无淋巴结转移者明显升高(t=6.10,P<0.01)。病例组患者不同年龄、病理类型、肿瘤大小及淋巴结有无转移之间外周血CD3^+和CD8^+水平的比较,均无明显差异(t值分别为0.94、0.48、0.76、0.51、0.57、0.38、0.15、0.59,均P>0.05)。经Pearson相关性分析结果显示,乳腺癌患者外周血中PLT与CD4^+的水平存在负相关关系(r=-0.52,P<0.01),而与CD3^+、CD8^+水平并无显著关系(r值分别为-0.11、0.09,均P>0.05)。结论乳腺癌患者外周血中存在PLT和CD8^+高水平、CD3^+和CD4^+低水平的特点,PLT与CD4^+的水平存在负相关关系,且PLT与乳腺癌患者淋巴结转移存在一定关系。Objective To analyze the correlation between platelet(PLT),CD3^+,CD4^+,CD8^+lymphocyte levels in peripheral blood and clinicopathological features of breast cancer.Methods Eighty-five female patients with primary invasive breast cancer were selected as case group and 30 healthy female volunteers were selected as control group from January 2016 to March 2018 in Shaoxing Hospital of Traditional Chinese Medicine.The levels of PLT,CD3^+,CD4^+and CD8^+lymphocytes in peripheral blood of two groups were measured and compared.The relationship between peripheral blood PLT,CD3^+,CD4^+and CD8^+lymphocytes and clinicopathological features of breast cancer was analyzed and Pearson correlation was used to analyze the relationship between PLT and CD3^+,CD4^+and CD8^+lymphocyte levels in peripheral blood of breast cancer.Results The levels of PLT [(247.80±66.85)×109/L]and CD8^+ [(30.85±4.99)%]in peripheral blood of the case group increased significantly in comparison of those in the control group,but the levels of CD3^+[(56.70±6.93)%]and CD4^+[(24.64±4.84)%]were significantly lower in comparison of those in the control group[(118.97±16.35)×109/L,(23.05±3.14)%,(65.77±7.35)%,(31.90±3.25)%](t value was 7.37,6.07,7.62 and 8.01,respectively,all P<0.01).There was no significant difference in PLT and CD4^+levels among cases of different ages,pathological types and tumor sizes(t value was 0.60,0.22,0.36,0.51,0.39 and,0.27,respectively.all P >0.05).The level of PLT in peripheral blood of patients with lymph node metastasis was significantly higher than that of patients without lymph node metastasis(t=6.10,P<0.01).There were no significant differences in peripheral blood CD3^+and CD8^+levels among patients of different ages,pathological types,tumor sizes and lymph node metastasis condition(t value was 0.94,0.48,0.76,0.51,0.57,0.38,0.15 and 0.59,respectively,all P >0.05).Pearson correlation analysis showed that there was a negative correlation between PLT and CD4^+levels in peripheral blood of breast cancer(r=-0.52,P<0.01),but th
