摘  要:目的分析腔内治疗和单纯压力治疗对髂股静脉血栓形成后综合征(PTS)中期疗效的影响。方法回顾性分析2013年1月至2015年12月107例髂股静脉PTS患者的临床资料,腔内治疗患者(58例)给予球囊扩张、支架植入术,单纯压力治疗患者(49例)给予压力袜或弹力绷带压力治疗,观察两组患者临床治疗效果和支架植入后的中期通畅率。结果治疗技术成功率为100%。术后随访(27.2±11.3)个月,腔内治疗一期通畅率为75.9%,二期通畅率为87.9%。腔内治疗组和单纯压力治疗组患者临床严重程度评分、Villalta评分和水肿评分较术前均有明显的下降,但腔内治疗组下降更明显(P<0.05)。疼痛评分腔内治疗组好转明显,单纯压力治疗组无明显变化。结论腔内治疗髂股静脉PTS安全、有效。和单纯压力治疗相比,腔内治疗有助于改善患者的临床症状。ObjectiveTo compare the mid-term effects of endovascular treatment and compression therapy in patients with iliofemoral vein post-thrombosis syndrome (PTS).MethodsFrom January 2013 to December 2015, 107 patients with iliofemoral PTS treated in our hospital were retrospectively analyzed. Endovascular treatment (ET) group (58 patients) received stent implantation after balloon expansion. Elastic compression stockings or elastic bandages were used in compression therapy (CPT) group (49 patients). Clinical outcomes were evaluated in two groups. Mid-term stent patency rate was also assessed.ResultsTechnical success rate was 100%. With the follow up of (27.2±11.3) months, the primary and secondary patency rate were 75.9% and 87.9%, respectively. There was significant decrease of VCSS, Villalta and edema scores in both groups. However, a significantly greater decrease was observed only in ET group (P<0.05) . Pain score decreased significantly in ET group and was not greatly changed in CPT group.ConclusionsEndovascular treatment of iliofemoral PTS is safe and feasible. Compared with compression therapy, endovascular treatment has advantages in improving clinical results.
