摘　　要：目的探讨早期异位妊娠患者血清人绒毛膜促性腺激素(β-HCG)、雌激素(E2)、孕酮(P)的水平变化及其预测价值。方法选取该院(2016年01月-2017年12月)确诊的90例异位妊娠患者(病例组)、同期正常妊娠妇女90例作为对照组,检测两组的血清β-HCG、E2和P水平,并采用ROC曲线求取最佳诊断界值所对应的灵敏度、特异度等诊断学指标。结果病例组的血清β-HCG、E2和P水平均显著的低于对照组,差异具有统计学意义(P <0. 05);血清β-HCG预测异位妊娠的灵敏度为88. 18%、特异度为77. 03%、ROC曲线下面积AUC值为0. 800;血清E2预测异位妊娠的灵敏度为84. 29%、特异度为72. 06%、ROC曲线下面积AUC值为0. 777;血清P预测异位妊娠的灵敏度为82. 09%、特异度为71. 55%、ROC曲线下面积AUC值为0. 767;血清β-HCG、E2和P水平联合检测预测异位妊娠的灵敏度为95. 09%、特异度为90. 63%%、ROC曲线下面积AUC值为0. 936。结论早期异位妊娠患者血清β-HCG、E2、P的水平较低。Objective To investigate the changes and predictive values of serum human chorionic gonadotropin(beta -HCG), estrogen(E2)and progesterone(P)in patients with early ectopic pregnancy.Methods 90 cases of ectopic pregnancy (January 2016 to December 2017) were selected as the case group and 90 cases of normal pregnant women were selected as the control group.The levels of serum beta -HCG,E2 and P were detected in the two groups,and the ROC curve was used to determine the sensitivity and specificity of the best diagnostic value.Results The average serum levels of serum beta -HCG,E2and P in the case group were significantly lower than those in the control group(P <0.05).The sensitivity of serum beta -HCG to predict ectopic pregnancy was 88.18%,the specificity was 77.03%,and the AUC value under the ROC curve was 0.800.The sensitivity of the serum E2 to predict ectopic pregnancy was 84.29%,the specificity was 72.06%,and the area AUC under the ROC curve was 0.777.The sensitivity of the serum P to predict ectopic pregnancy was 82.09%,the specificity was 71.55%, and the area AUC under the ROC curve was 0.767.The sensitivity of the combined detection of serum beta -HCG,E2 and P to predict ectopic pregnancy was 95.09%,the specificity was 90.63%,and the AUC value under the ROC curve was 0.936.Conclusion The serum levels of serum beta -HCG,E2 and P in patients with early ectopic pregnancy are relatively low and the combined application of the three methods has high clinical value in the early diagnosis of ectopic pregnancy.
