摘  要:目的分析各年级中小学生的血尿酸水平及相关影响因素,为采取有效干预措施提供参考。方法采用多阶段分层整群抽样方法,根据供餐方式和规模抽取朝阳区小学一、三、五年级和初中一年级共8所学校557名学生作为研究对象,进行问卷调查、体格检查和血液检测。结果学生血尿酸平均水平为(296.2±77.4)μmol/L,男生血尿酸水平[(304.9±82.7)μmol/L]高于女生[(286.6±70.2)μmol/L],差异有统计学意义(t=2.80,P=0.01);不同年级间血尿酸水平差异有统计学意义(F=47.75,P<0.01)。研究对象高尿酸血症患病率为7.0%,性别之间差异无统计学意义(χ^2=1.25,P=0.15),初一年级(21.4%)高于小学五年级(6.2%)和一、三年级(均为0)(χ^2=65.31,P<0.01)。Logistic回归分析发现,高年龄组和高同型半胱氨酸血症(Hhcy)与高尿酸血症呈负相关,而每天足够多的睡眠时间与其呈正相关(P值均<0.05)。结论中小学生血尿酸水平较高,男生和高年级学生是血尿酸水平关注的重点人群,高年级和高同型半胱氨酸血症是学生高尿酸血症的重点关注指标。Objective This study aimed to investigate serum uric acid concentration and risk factors among primary and middle school students in Chaoyang district of Beijing. Methods By using multistage clustering sampling method,a total of 557 students in grade 1,grade 3,grade 5 and grade 7 from 8 schools in Chaoyang district of Beijing were selected. Questionnaire survey,physical examination and blood tests were implemented. Results Average serum uric acid level was 296.2±77.4 μmol/L,with male( 304.9±82.7) μmol/L higher than female students( 286.6±70.2) μmol/L( t = 2.80,P = 0.01). Serum uric acid differed significantly across grade( F = 47.75,P<0.01). The prevalence of hyperuricemia was 7.0%,with no sex difference,with grade 7( 21.4%) higher than grade 5( 6.2%),grade 1 and grade 3( 0)( χ2= 65.31,P<0.01). High age group and hyperhomocysteinemia( Hhcy) associated with higher risk for HUA,and sufficient sleep duration was negatively associated with HUA risk. Conclusions High level of serum uric acid concentration is observed among primary and secondary school students in Chaoyang district of Beijing,especially among males and older adolescents with hyperhomocysteinemia.
