磷霉素氨丁三醇对尿路致病性大肠埃希菌的体外抗菌活性研究

摘  要:目的了解磷霉素氨丁三醇对尿路感染患者大肠埃希菌的体外抗菌活性,为临床诊治提供参考依据。方法采用VITEK2 Compact进行细菌鉴定,药物敏感性试验采用VITEK2 Compact或纸片扩散(K-B)法,按美国临床和实验室标准化协会2016年标准判读药物敏感性试验结果,用WHONET 5.6软件进行数据统计分析。结果 1 125株病原菌中共分离出522株大肠埃希菌,其中产超广谱β-内酰胺酶(ESBLs)的大肠埃希菌251株(48.1%),其对磷霉素氨丁三醇的敏感率为96.6%;非产ESBLs大肠埃希菌271株(51.9%),其对磷霉素氨丁三醇的敏感率为99.6%。大肠埃希菌对碳青霉烯类抗菌药物(亚胺培南、厄他培南)均保持很好的敏感性(>99%),产ESBLs菌株对大多数抗菌药物的耐药率明显高于非产ESBLs菌株。结论尿路感染以大肠埃希菌为主,磷霉素氨丁三醇对产ESBLs的大肠埃希菌具有良好的体外抗菌活性。Objective To investigate the in vitro activities of antimicrobial agents including fosfomycin tromethamine against Escherihia coli isolated from patients with urinary tract infection(UTI) and provide the basis for clinical diagnosis and treatment. Methods Bacteria from clinical specimen were identified by VITEK2 Compact, and drug sensitive tests were tested by VITEK2 Compact or disk diffusion method(K-B method). All data were analyzed by WHONET 5.6 statistical software according to the Clinical and Laboratory Standards Institute(CLSI) 2016 breakpoints. Results A total of the 522 Escherichia coli strains were isolated; extended-spectrum-beta-lactamases(ESBLs) producing strains accounted for 251(48.1%),and the susceptibility rate to fosfomycin tromethamine was 96.6%. The non-ESBLs-producing Escherichia coli strains were 271(51.9%); the susceptibility rate to fosfomycin tromethamine was 99.6%. Escherichia coli were still highly susceptible to carbapenems( >99%). The ESBLs-producing isolates were significantly more resistant to most antibiotics than the corresponding non-ESBLs-producing isolates. Conclusion Escherichia coli was the most popular strains isolated from urine samples. Fosfomycin tromethamine has good antibacterial in vitro activity against ESBLs-producing Escherichia coli.
