摘 要:目的了解福建省耐多药结核分枝杆菌的基因多态性和主要流行基因型,为耐多药结核病的传播防控提供参考。方法回顾性分析福建省耐药监测数据,收集标本并采用WHO推荐的方法进行药敏试验检出的耐多药结核分枝杆菌119株,采用24位点VNTR(24-VNTR)和间隔区寡核苷酸分型技术(Spoligotyping)进行基因分型,实验数据及结果数字化后录入Excel,在网站http://www.miru-vntrplus.org进行比对,分型结果通过BioNumerics5.0软件进行聚类分析。结果 119株MDR菌株经24-VNTR分型呈现112种基因型(105个独特基因型,7个基因簇),成簇率为11.76%。24-VNTR对119株菌的分辨力(HGDI)为0.999。经Spoligotyping分型,北京基因型占67.23%,非北京基因型占32.77%,以H家族,T家族常见。共26个基因型(19个独特基因型和7个簇),成簇率为84.03%(每簇含2~75株)。Spoligotyping对119株菌的Hunter-Gaston指数为0.597。结论福建省耐多药结核分枝杆菌主要流行型为北京基因型,具有基因多态性。Objective To ascertain the genetic diversity and main genotypes of multi-drug-resistant(MDR)Mycobacterium tuberculosis isolates from Fujian Province in order to provide a reference for tuberculosis(TB)control. Methods Data on surveillance of drug resistance were analyzed retrospectively.Strains from drug resistance surveillance sites were collected,and DST was conducted as recommended by the WHO.One hundred and nineteen MDR M.tuberculosis strains were isolated and genotyped using 24-VNTR and spoligotyping.The digital results were input in Excel and aligned in http://www.miru-vntrplus.org,clustered using BioNumerics 5.0,and analyzed using the statistical software SPSS13.0. Results The 119 MDR strains represented 112 genotypes according to VNTR,including105 strains with 105 unique genotypes and 7clusters.The clustering rate was 11.76%.The HGDI of 24-VNTR for the 119 strains was 0.999.Strains of the Beijing genotype accounted for 67.23% of all strains according to spoligotyping,while strains not of the Beijing genotype accounted for 32.77%.Strains not of the Beijing genotype often belonged to the H or and T family.In total,26 genotypes including 19 unique genotypes and 7clusters,were identified using spoligotyping.The clustering rate was 84.03%(with 2-75 isolates in each cluster).The HGDI of spoligotyping was 0.597. Conclusion MDR M.tuberculosis strains in Fujian Province are mainly of the Beijing genotype,and they display genetic diversity.
