摘  要:目的建立孕妇弓形虫感染的辅助诊断新方法,并筛选弓形虫感染血清标志物。方法利用基质辅助激光解吸电离飞行时间质谱(MALDI-TOF-MS)结合临床ClinProTool分析,将弓形虫IgG抗体阳性组与健康对照人群组进行比较,根据血清差异表达多肽建立弓形虫感染诊断方法,并验证其准确性。结果与健康对照组相比,弓形虫IgG抗体阳性组血清m/z 3 639.4、8 125.51、8 139.24、3 254.51、3 238.39、2 656.91、7 765.07、2 078.03、1 940.3高表达,在m/z 3 954.25、4 269.09、2 750.17、6 447.53、1 735.96、6 645.92、4 280.84低表达,根据差异表达多肽,利用ClinProTool建立的诊断方法对20例弓形虫IgG抗体阳性及15例IgM抗体阳性人群血清进行验证,准确率100%。结论利用血清差异多肽谱结合ClinProTool建立的诊断方法准确率高,有望用于人群弓形虫感染筛查。Objectives To devise a new technique to diagnose Toxoplasma gondii infection in pregnant women and to screen serum markers. Methods Serum samples were collected from pregnant women with toxoplasmosis and healthy controls.Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry(MALDI-TOF-MS)was combined with analysis using ClinProTool to distinguish the IgG-positive group infected with T.gondii and the healthy controls.A diagnostic technique was devised based on the differential expression of peptides in serum,and its accuracy was verified. Results Compared to the healthy control group,the IgG-positive group infected with T.gondii had significantly increased expression of peptides with an m/z of 3,639.4,8,125.51,8,139.24,3,254.51,3,238.39,2,656.91,7,765.07,2,078.03,and 1,940.3but significantly decreased expression of peptides with an m/z of 3,954.25,4,269.09,2,750.17,6,447.53,1,735.96,6,645.92,and 4,280.84.A diagnostic technique based on differential peptide peaks was created using ClinProTool.Its diagnostic accuracy was 100%in 20 Toxoplasma IgG-positive patients and 15IgM-positive patients. Conclusion This diagnostic technique using serum differential peptides combined with ClinProTool is expected to serve as a new technique to screen for T.gondii infection.
