摘　　要：慢性萎缩性胃炎(CAG)属于慢性胃炎的一种类型,表现为胃固有腺体减少、萎缩,伴或不伴纤维替代、肠腺化生和(或)假幽门腺化生,其病因及发病机制复杂,目前尚未完全明确,是现今消化领域的常见病及难治病。CAG被认为是胃癌的癌前疾病,因此积极防治CAG对预防胃癌意义重大。胃黏膜损伤在CAG发生发展过程中起重要作用,COX-2高表达、p53基因发生突变参与到CAG发病的病理过程中,COX-2、p53表达的变化和胃黏膜损伤的关系机制已经成为近年的研究热点。现对CAG的发病机制进行新的探讨,并综述近年来中医药治疗CAG的研究进展,为临床上治疗CAG提供新思路。Chronic atrophic gastritis (CAG)is a type of chronic gastritis.It is characterized by the reduction and atrophy of the glandular glands of the stomach,with or without fibrous substitution,metaplasia of the intestinal glands and (or)false metaplasia of the pylorus.The etiology and pathogenesis of CAG is still unclear.It is a common and hard disease in the field of digestive system.It is regarded that CAG is the pre-caneerrous condition of gastric cancer,so to treat CAG is very important.The lesion of gastric mueosa plays an important role in the development of CAG.The high expression of COX-2 and the mutation of p53 gene are involved in the pathogenesis of CAG.The relation of gastric mueosa lesion and the expression of COX-2 and p53 became research hotspots in recent years.This article explored a new mechanism of CAG's pathogenesis,and reviewed the research progress in treatment of CAG with TCM,opening a new way for the clinical treatment of CAG.
