人输精管内滤过装置(IVD)植入后远期并发症原因分析及诊疗经验

摘　　要：目的探讨人输精管内滤过装置(IVD)植入后远期并发症发生原因及诊治措施,为今后研究提供参考。方法对2012年8月至2016年1月期间我院共收治的16例IVD并发症病例的临床资料(其中7例采取手术治疗,9例采用保守治疗)进行回顾性研究。结果 7例手术基本成功,术后均获随访。1年随访情况如下:(1)精液检查1例精子浓度>20×106/m L,6例检查结果均为严重少精子症(其中1例术后2个月后反复精液检查均报告为无精子);(2)症状改善情况阴囊疼痛症状基本消失,但血精无明显改善;(3)切除的输精管组织病理报告:管壁平滑肌及纤维组织增生,黏膜间质见少量淋巴组织浸润。9例经保守治疗后,症状有所改善,但易反复。结论定期有效的随访有利于IVD植入术后并发症的防治,人输精管内滤过装置植入前需要进行前瞻性研究并对远期并发症进行评估。Objective To discuss the causes and treatment of long term complications after intra-vasal device(IVD) implantation. Methods Clinical data of 16 cases with IVD complication in our hospital from August 2012 to January 2016(7 of them with surgical treatment and 9 with conservative treatment) were retrospectively analyzed. Results 7 cases with IVD complication were successfully treated by surgery, and were followed up for 1 year:(1) Semen examination: Sperm concentration of 1 cases were over 20×106/mL;other 6 cases were all severe oligozoospermia(no sperm were detected for one case after 2 months);(2) Symptoms: The symptom of scrotal pain disappeared, but no significant improvement in hemospermia;(3) Pathological report: wall smooth muscle and fibrous tissue proliferation in the tube wall, with a small amount of lymphoid tissue infiltration. 9 cases with IVD complication were treated by conservative treatment, the symptoms were improved, but easy to relapse. Conclusion Regular and effective follow-up is in favor of prevention and treatment of adverse reaction of IVD, A prospective study is needed for a human intra-vasal device before implantation and its long-term complications also to be evaluated.
