摘　　要：目的调查糖尿病患者出院后健康管理现状及对糖尿病相关知识的知晓和掌握程度,为糖尿病的社区健康管理提供科学依据。方法随机抽取2015-2016年住院并确诊的糖尿病患者120例,采用自行设计问卷对其出院后现况进行调查。结果女性患者在血糖和血压监测、吸烟饮酒等危险行为和高油高盐等生活方式改变方面明显高于男性患者(P<0.05),空腹血糖控制的患者在自测血压、控烟限酒及限盐等方面明显高于血糖未控制组(P<0.05),血糖异常组肥胖患者血糖值明显高于体重正常和超重患者(P<0.05);知晓血糖控制目标的患者占37.9%,知晓糖尿病并发症及其危害的患者仅占25.7%,60.5%的患者对现在的生活不满意。结论2型糖尿病患者出院后对糖尿病相关知识了解甚少,需要县级医疗机构强化糖尿病患者住院期间的健康教育,加强糖尿病专科团队与社区责任医生进行有效对接,进一步提升社区卫生服务机构糖尿病健康管理能力建设。Objective To investigate the situation of health management of diabetic patients after discharged,the awareness and mastery of diabetes related knowledge,so as to provide scientific basis for the community health management of diabetes mellitus.Methods A total of 120 hospitalized and diagnosed diabetic patients during 2015 and 2016 were randomly selected.A self-designed questionnaire was used to investigate the current situation after discharge.Results Women were significantly higher than men in terms of blood glucose and blood pressure monitoring,risky behaviors such as smoking and drinking,and lifestyle changes such as high oil and salt(P<0.05).The patients with fasting blood glucose control were significantly higher than the uncontrolled blood glucose control group (P<0.05)in the self-measured blood pressure,smoking control limit and salt restriction (P<0.05).The blood glucose level of obese patients was significantly higher than that of normal weight and overweight patients (P<0.05);the awareness of blood glucose control targets was 37.9%,and only 25. 7% of the diabetes complications and their hazards were known,and 60.5% of the patients were dissatisfied with the present life.Conclusion Type 2 diabetes patients have little knowledge about diabetes related knowledge,it is necessary to need county-level medical institutions to strengthen health education during the period of diabetes in hospital,strengthen the diabetes specialist team and community responsible doctors to carry out effective docking,and further improve the health management ability of community health service agencies.
