清热消肿法对高位复杂性肛瘘术后伤口愈合及肛门压力影响的临床观察

摘　　要：目的观察清热消肿法在复杂性肛瘘术后恢复过程中的作用。方法将2014年4月—2017年10月安徽医科大学第一附属医院痔瘘科60例复杂性肛瘘术后住院患者随机分为治疗组和对照组,每组各30例。治疗组在术后予以清热消肿中药熏洗7 d,对照组使用高锰酸钾熏洗7 d。2组其余治疗和处理方法相同。治疗7 d观察各组病例术后熏洗前及术后10 d创面宽度、深度,肛门疼痛、水肿,创面分泌物、肉芽生长等改善情况,并对各组病例术前及术后30 d的肛管最大收缩压、肛管静息压、肛管最长收缩时间、直肠静息压进行检测。结果治疗组术后10 d伤口愈合积分明显优于术后处理前(P <0. 01); 2组比较,治疗组优于对照组(P <0. 05,P <0. 01)。术前和术后30 d比较,肛门功能受一定程度的影响,但治疗组相较于对照组,能够有效地降低这种影响(P <0. 05)。结论清热消肿中药与高锰酸钾比较,可明显促进创面愈合,并能改善肛门的功能。Objective To observe the effect of clearing heat for detumescence method on reply after surgery of complicated anal fistula.Methods From April 2014 to October 2017,60 cases of complicated anal fistula patients were randomly divided into treatment group and control group in the first affiliated hospital of Anhui Medical University,with 30 patients in each.Based on the same treatment,the treatment group was fumigated with Chinese medicine which have the effect of clearing heat for detumescence,while the control group was fumigated bypotassium permanganate.To observe the improvement of wound width,depth,anal pain,edema,surface secretions and granulation growth in 10 before and after postoperative treatment in each group.The maximum systolic pressure of anal canal,resting pressure of anal canal,maximum contraction time of anal canal,and resting pressure of rectum were tested before and after surgery.Results The wound healing points of day 10 after operation were superior to those before operation (P <0.01 ).The treatment group was superior to the control group (P <0.05,P < 0.01 ).30 days after surgery,anal function was improved significantly than that before surgery,and the treatment group was superior to the control group.(P <O.05).Conclusion Compared with potassium permanganate,Chinese medicine with the function of clearing heat for detumescence could significantly promote wound healing and improve the function of anus.
