原发性失眠患者生活质量及危险因素的研究

摘　　要：目的探讨原发性失眠患者生活质量及其相关危险因素。方法采用WHOQOL分别对50例原发性失眠症患者和40名健康对照组进行生活质量(QOL)的评估;应用PSQI、SDS、SAS评定原发性失眠症患者的睡眠质量及焦虑抑郁程度;采用独立样本t检验、卡方检验、及多元逐步回归分析比较2组的差异性及生活质量相关危险因素。结果原发性失眠组在生理领域、心理领域、独立性领域、社会关系领域、环境领域和总体生活质量中评分均低于健康对照组(P<0.05)。在多元逐步回归分析中,SDS分、PSQI分、家族史等3个自变量成为影响患者生活质量总分的主要因素。结论原发性失眠症患者的生活质量在生理、心理、独立性、社会关系、环境等维度和总体生活质量都有显著降低。失眠所伴随的抑郁情绪,失眠的严重程度和有无家族史是影响原发性失眠症患者生活质量的主要因素。Objective To explorethe quality of life and risk factors in patients with primary insomnia. Methods 50 patients with primary insomnia(PI),well matched with 40 healthy controls(HC)in age,gender and education,were assessed by WHOQOL,PSQI,SAS and SDS. The difference between groups was analyzed byindependent-samples t test or χ2 test,Meanwhile,multiple linear regression analysis were performed to further assess the factors affecting the quality of life in patients withprimary insomnia. Results Compared with the healthy controls,the WHOQOL in the physical,psychological,independent,social relationships,environment domains and the overall QOL weresignificantly lower in the PI patients(P<0.05). Multiple linearregression analysesindicated that the depression symptom,severity of insomnia and family history weresignificant predictors of impairment of quality of life in PI patients. Conclusions In PI patients,the quality of life in the physical,psychological,independent,social relationships,environment domains and the overall QOL wereimpaired. The severity of insomnia,family history,depressive symptomsare the important factors resulting in impairment of quality of life in PI patients.
