中心静脉导管闭式引流治疗原发自发性气胸的临床观察

摘　　要：目的探讨中心静脉导管闭式引流治疗原发自发性气胸的应用效果。方法 150例原发自发性气胸患者,随机分为对照组与观察组,每组75例。对照组采用中心静脉导管接负压瓶闭式引流,观察组给予中心静脉导管接水封瓶闭式引流。比较两组患者临床疗效、平均复发时间、平均住院时间、平均住院费用及并发症发生情况。结果观察组总有效率94.67%显著高于对照组的84.00%,平均复发时间(34.5±8.6)h显著短于对照组的(49.4±12.1)h,平均住院时间(37.6±10.2)h显著短于对照组的(57.2±11.6)h,平均住院费用(1037.5±98.6)元显著少于对照组的(1344.8±123.5)元,差异有统计学意义(P<0.05)。观察组患者并发症发生率5.33%与对照组的9.33%比较,差异无统计学意义(P>0.05)。结论采用中心静脉导管闭式引流治疗原发自发性气胸的疗效显著,可缩短住院时间、减少治疗费用,值得临床推广应用。Objective To discuss the application effect of central venous catheter closed drainage for primary spontaneous pneumothorax. Methods A total of 150 primary spontaneous pneumothorax patients were randomly divided into control group and observation group, with 75 cases in each group. The control group was treated with central venous catheter closed drainage with water-sealed bottle, and the observation group was treated with central venous catheter with water-sealed closure. Comparison were made on clinical efficacy, mean recurrence time, mean hospitalization time, mean hospitalization costs and occurrence of complications between the two groups. Results The observation group had significantly higher total effective rate as 94.67% than 84.00% in the control group, and shorter mean recurrence time as(34.5±8.6) h than(49.4±12.1) h in the control group, and shorter mean hospitalization time as(37.6±10.2) h than(57.2±11.6) h in the control group, and less mean hospitalization time as(1037.5±98.6) yuan than(1344.8±123.5) yuan in the control group, and the difference was statistically significant(P<0.05). The observation group had no statistically significant difference in incidence of complications as 5.33%, comparing with 9.33% in the control group(P >0.05). Conclusion Central venous catheter closed drainage shows remarkable efficacy in treating primary spontaneous pneumothorax, and it can shorten hospitalization time and reduce treatment costs. It is worthy of clinical promotion and application.
