摘　　要：目的探讨T2WI 3D容积扫描(不抑脂)在肛管直肠瘘术前诊断的应用价值。方法搜集85例临床诊断肛瘘患者,常规T1WI轴位,T2WI抑脂轴位、矢状位、冠状位,T2WI 3D容积扫描(不抑脂)冠状位,并进行多平面重组,检查瘘管和内口情况。阅片结果与手术结果对照,比较瘘管和内口常规T2WI抑脂序列检查结果手术符合率及T2WI 3D容积扫描(不抑脂)后重组检查结果手术符合率。结果 T2WI 3D容积扫描(不抑脂)瘘管检查结果与手术结果的一致率达到91.76%,内口检出结果与手术结果一致率达88.23%。常规T2WI抑脂序列瘘管检查结果与手术结果符合率为70.59%,内口检出结果与手术结果一致率为69.41%。结论在肛管直肠瘘患者中,T2WI3D容积扫描(不抑脂)瘘管、内口检查结果与手术结果的符合率较常规T2WI抑脂序列明显增高,能为临床提供更好的手术依据。Objective To discuss the value of T2WI 3D volume scan(non fat pressing)for the preoperative diagnosis of anorectal fistula.Methods Collect 85 cases of clinical diagnosis of anal fistula,the routine T1WI axis,T2WI FATSAT axis,sagittal,coronal,and T2WI 3D volume scan(non fat pressing),and carry out multiple planar reconstruction,inspect fistula and internal mouth condition.Comparison between the results of the film reading and the results of the operation.Results The consistent rate of fistula between the T2WI 3D volume scan(non fat pressing)examination and operation results reached 91.76%,and the consistent rate of the inner mouth detection and operation results was 88.23%.The coincidence rate of fistula between the results of the routine T2WI FATSAT sequence and the results of the operation was70.59%,and the rate of consistency between the results of the inner mouth and the results of the operation was 69.41%.Conclusion In the patients with anorectal fistula,the coincidence rate of the T2WI 3D volume scan(non fat pressing)was significantly higher than that of the conventional T2WI FATSAT sequence.The T2WI 3D volume scan(non fat pressing)sequence could provide a good basis for the clinical operation.
