摘 要:目的探讨X线三线比值在诊断3个月以下婴儿发育性髋关节脱位(DDH)的价值。方法回顾性分析2012年5月至2017年5月于本院就诊,临床怀疑为DDH的85例患儿的临床资料。采用超声Graf法结合MRI进行分型,并在X线片中以股骨近端干骺端的中点(记为O点)为起点,分别做至髋臼外上缘的连线(a线)、至Y型软骨中心连线(b线)以及髋臼下缘的连线(c线),分别计算a/b、b/c和a/c的比值,分析髋关节三线比值的差异。结果 85例患儿(170髋)中正常髋关节84髋,发育不良髋关节36髋,脱位髋关节50髋。正常髋关节中a/b、b/c和a/c的比值接近于1,分别为1.01±0.03、0.99±0.03、1.00±0.04;髋关节发育不良的a/b和a/c的值 > 1,b/c值接近于1,分别为1.09±0.04、1.11±0.05、1.02±0.09;髋关节脱位的a/b和a/c的值<1,b/c值接近于1,分别为0.95±0.03、0.91±0.06、0.98±0.04,髋关节发育不良的a/b和a/c的值显著大于正常髋关节,差异具有统计学意义(P<0.05),髋关节脱位的a/b和a/c的值显著小于正常髋关节,差异同样有统计学意义(P<0.05)。结论X线三线比值能够为3个月以下婴儿DDH的早期诊断提供科学参考,具有一定的临床推广价值。Objective To explore the value of digital radiography with ratio of three lines in the diagnosis of developmental dislocation of the hip in infants less than three months old.Methods 85 infants less than three months with suspected developmental dislocation of the hip were retrospectively reviewed.Three lines were measured from the midpoint of the femoral metaphysis to upper margin(line a),lower margin(line b) and Y-shaped cartilage center of the acetabulum(line c),respectively.The values of a/b,b/c and a/c were calculated.Then the relationship between the ratio of three lines and dislocation types of developmental dislocation of the hip were investigated.Results 85 infants(n = 170 hips)included normal hips(n=84 hips),dysplasia(n=36 hips) and dislocation(n=50 hips).The values of a/b,b/c and a/c were 1 for normal hips,in which were(1.01±0.03),(0.99±0.03),(1.00±0.04).The values of a/b and a/c were more than 1 for dysplasia,in which were(1.09±0.04),(1.11±0.05).The values of a/b and a/c were less than 1 for dislocation,in which were(0.95±0.03),(0.91±0.06).The values of a/b and a/c for dysplasia were more than that for normal hips,and the differences were statistically significant(P<0.05).The values of a/b and a/c for dysplasia were less than that for normal hips,and the difference were statistically significant(P<0.05).Conclusion Digital radiography with ratio of three lines provides a new method for diagnosis of developmental dislocation of the hip in infants less than three month old.
