孟鲁司特联合激素对哮喘患儿外周血CD_4^+CD_(25)^+Treg、IL-17水平的影响

摘　　要：目的探讨孟鲁司特联合激素治疗对哮喘患儿外周血CD_4^+CD_(25)^+调节性T细胞(Treg)、血清白介素-17 (IL-17)水平的影响,为哮喘的治疗提供理论依据。方法选取2015年6月-2017年6月成都市郫都区人民医院收治的60例哮喘患儿为研究对象,根据随机数字表法分为对照组和观察组,每组各30例。对照组给予吸入型糖皮质激素布地奈德治疗,观察组在对照组基础上联合孟鲁司特治疗。比较两组临床疗效、治疗前后肺功能[第1秒用力呼气容积(FEV1)、最大呼气峰流速(PEF)]指标,外周血CD_4^+CD_(25)^+Treg、IL-17水平,并观察药物不良反应发生情况。结果观察组治疗总有效率显著高于对照组,差异均有统计学意义(均P<0. 05)。治疗后,两组FEV1、PEF、外周血CD_4^+CD_(25)^+Treg均显著高于治疗前,IL-17水平低于治疗前,差异均有统计学意义(均P<0. 05);且观察组FEV1、PEF、外周血CD_4^+CD_(25)^+Treg均高于对照组,IL-17水平低于对照组,差异均有统计学意义(均P<0. 05)。两组药物不良反应发生率比较,差异无统计学意义(P>0. 05)。结论孟鲁司特联合布地奈德治疗儿童哮喘疗效明确,可明显改善患儿肺功能,上调外周血CD_4^+CD_(25)^+Treg水平,降低IL-17水平。Objective To explore the effect of montelukast combined with hormone on the levels of CD4+CD25+Treg and interleukin-17(IL-17) in peripheral blood of children with asthma,provide a theoretical basis for treatment of asthma.Methods Sixty children with asthma treated in People's Hospital of Pidu District from June 2015 to June 2017 were selected and divided into control group and observation group according to random number table,30 children in each group.The children in control group were treated by inhalation therapy of budesonide,while the children in observation group were treated by inhalation therapy of budesonide and montelukast.The clinical effects and lung function indexes [forced expiratory volume in 1 second(FEV1),peak expiratory flow(PEF) ] before and after treatment,CD4+CD25+Treg and IL-17 levels in peripheral blood were compared between the two groups,and adverse drug reactions were observed.Results The total effective rate in observation group was statistically significantly higher than that in control group(P<0.05).After treatment,FEV1,PEF,and CD4^+CD25^+Treg in peripheral blood in the two groups increased significantly,while IL-17 levels decreased significantly(P<0.05).After treatment,FEV1,PEF,and CD4^+CD25^+Treg in peripheral blood in observation group were statistically significantly higher than those in control group,while IL-17 level in peripheral blood in observation group was statistically significantly lower than that in control group(P<0.05).There was no statistically significant difference in the incidence rate of adverse drug reactions between the two groups(P >0.05).Conclusion Montelukast combined with budesonide is effective in treating children with asthma.The regimen can significantly improve lung function,increase CD4+CD25+Treg in peripheral blood and decrease IL-17 level.
